Q-TC离散度测定及冠心病及高血压心脏病关系   【摘要】 目的 通过观察健康人群、冠心病及高血压心脏病的Q-TC离散度(Q-Tcd),以探讨Q-Tcd对冠心病及高血压心脏病的应用价值。方法 回顾50例冠心病和50例高血压心脏病12导联ECG的Q-Tcd,并与100例正常人的Q-Tcd进行对比。结果 正常组男、女之间Q-Tc有显著性差异(Plt;0.01),平均女性比男性长18 ms;但Q-Tcd无显著性差异(Pgt;0.05)。冠心病组和高心病组的Q-Tcmin接近于正常组(395.9±24.6)ms,而Q-Tcmax均明显大于正常组(418.5±24.7)ms;Q-Tcd冠心病组和高心病组[(50.3±17.0)ms、(48.1±18.3)ms]明显大于正常组[(22.7±8.1)ms,Plt;0.001]。结论 Q-Tcd增大是冠心病和高血压心脏病的重要心电学指标,同时也是二者判断病情预后和预告严重室性心律失常简明有用的心电学指标。 【关键词】 心电描记术;冠心病;高血压心脏病;Q-T间期;离散度;室性心律失常 [Abstract] Objective To investigate practical value of Q-TC dispersion(Q-Tcd) in coronary heart disease and hypertensive heart disease by analyzing the differences in Q-Tcd among healthy population,coronary heart disease and hyperfensive heart disease patients.Methods A retrospecitve analysis was performed on 50 cases of coronary heart disease and 50 cases of hypertensive heart disease with regard to Q-Tcd on the 12-lead ECG,as compared with those of 100 healthy controls.Results There was signficant difference in Q-TC between the male and female of the control group(Plt;0.001),and the average Q-TC was 18 ms longer in the female than in the male.But there was no significant difference in Q-Tcd between the male and female of the control group(Pgt;0.05).The minimal Q-TC values of both coronary heart disease and hypertensive heart disease groups were approximal to those of the control group(395.9±24.6 ms),while the maximal Q-TC values were much longer(418.5±24.7 ms).The values of the Q-Tcd were considerably greater in both the coronary heart disease and hypertensive heart disease groups than in the cotrol group(50.3±17.0 ms vs.22.7±8.1 ms,and 48.1±18.3 ms vs. 22.7±8.1 ms,respectively),both Plt;0.001.Conclusion Increased Q-Tcd proves to be the important electrocardiological indicator for the coronary heart disease and hypertensive heart disease,and also the valuable indicator for judging the disease condition and prognosis,and predicting the severe ventricular arrhythmia of both two diseases.
