HEALTH, SAFETY AND ENVIRONMENTAL INFORMATION HSE QUESTIONNAIRE GENERAL GUIDELINES FOR THE COMPLETION OF THE HSE QUESTIONNAIRE This questionnaire covers the information required to assess the extent to which HSE and its management are organised by the Tenderer. The Tenderer is advised to cover all relevant activities, (including support), and not just those conducted on its work sites. Emphasis should be placed on providing complete answers. Little value will be placed on yes answers to the questions if not substantiated by supporting documentary evidence. The Tenderer is encouraged to identify where it exceeds Company requirements. ? A brief response to the questionnaire should be made in hard paper copy, plus an electronic version. Where requested, detailed material such as manuals, forms, schedules and standards should be submitted in electronic form only, together with notes, and any other information considered relevant.
