TheinfluenceofCYP3A4/5 and MDRI genepolymorphismon individualizedmedication of tacrolimus and therecovery ofhepatic and renal functionearlyafterlivertransplantationAbstract: bjective: ①To summarize the individual difference of bloodconcentrationanddosageoftacrolimusduringtheearlyperiodinpatients underwentlivertransplantation.②ToprovideguidanceofCYP3A4/5andMDRI genepolymorphismtotheindividualizedmedicationoftacrolimusduringtheearlyperiodafterlivertransplantation.weexploretherelationshipbetweengene polymorphismofCYP3A4/5,MDRIandtheindividualdifferenceofblood concentrationanddosageoftacrolimus,therecoveryofhepaticandrenalfunction duringearlyperiodafterlivertransplantation.Methods:Throughanalyzingthe38 livertransplantationpatientsinourhospital,whosebasic immunosuppressive agentistacrolimus,wedeterminethegenepolymorphismofCYP3A4/5 and MDRIwithgenechipmicrovibrationmethod.ThenweusedtheEMIT(enzyme multipliedimmunoassaytechnique)todeterminethewholebloodtrough concentrationoftacrolimusandcollectedthedosage(D),concentration(C),the correctedratioofC/D,serumcreatinine,alanineaminotransferase(ALT),total bilirubin,bloodcoagulationfunction(INR)ofthepatientsinthe1th,2th,4thweekafterthelivertransplantation.Results:①ThecorrectedratioofC/Dandtheoralrequirementoftacrolimusduringtheearlyperiodafterlivertransplantation variedconsiderablyindifferentindividual.②Amongthe38livertransplantation patients, there are 14 CYP3A4C/C, 19 CYP3A4C/T, 4 CYP3A4T/T and 1indetectableashypotheticalmissingdata.Theoccurrencefrequencyofeach genotypeis37.8%,51.4%,10.8%.Amongthethreegroups,CYP3A4C/Cgroup hasthelowestrequirementoftacrolimus(P0.05)andthehighestcorrectedC/D ratiointhe1thweekofpostoperation(P0.05).ThecorrectedC/Dratioof CYP3A4C/CgroupissignificantlyhigherthanCYP3A4C/T group(P0.05).There werenostatisticaldifferencesinthe4thweek.TherecoveryspeedofALTin CYP3A4C/CgroupwasslowerthantheCYP3A4C/Tinthe1thweekof postoperation(P0.05)whiletheCYP3A4C/Cgrouprecoveredslowerthan CYP3A4T/Tinthe4thweekafters
