精品论文 参考文献 采用锁定钢板治疗肱骨近端骨折的疗效 胡万成 莫令翔 张永亮 吕杰 王建江 张阳彬 赵宇来 （大邑县人民医院骨科 四川成都 611300） 【摘要】目的 探讨锁定钢板治疗肱骨近端骨折的临床疗效。方法 回顾性分析2008年5月～2011年6月收治肱骨近端骨折26例。按Neer分型：Ⅱ型19例，Ⅲ型7例。术中骨折复位后采用锁定钢板固定，有骨缺损者取自体髂骨植骨。术后患肢用三角巾悬吊，3周后功能锻炼。结果 26例均获得随访，随访时间8～17个月，平均11个月，无畸形愈合、感染、肱骨头坏死、内固定物松动断裂、肩关节撞击症、骨化性肌炎等。根据Neer评分，优10例，良13例，可2例，差1例，优良率88.5%。结论 采用锁定钢板固定肱骨近端II、III型骨折疗效满意，是治疗肱骨近端骨折的较好选择。 【关键词】肱骨近端 骨折 锁定钢板 【中图分类号】R687【文献标识码】A【文章编号】2095-1752（2013）03-0113-02 Locking plate on the treatment of proximal humerus fracture. Hu wangcheng，mo ling xiang，zhang yong liang，et al. （Department of Oethopaedics,The peoplersquo;s Hospital of da yi City of Sichuan Province 61000） 【Abstract】 Objective To observe the effect locking steel on the treatment of proximal humerus fracture. Methods The 26 cases who sffered from proximal humerus fracture were admitted from March 2008 to June 2011.Neer classification: Ⅱ 19 cases, Ⅲ 7 cases. The patients received treatment with autoallergic iliac bone grafted. Affected limb was suspled throught triangular bandage after operation, and the patients do functional exercises.. Result All cases wrer followed up from 8 months to 17 months (the means 11months), and there were no malunion, infection, necrosis of caput humeri, cinch and discrupt of internal fixation, articulatio humeri strike and ossifying myositis.On the basis of Neer score,Excellent in 10 cases, good in 13 cases, 2 cases, and poor in 1 case. The rate of Excellent and good is 88.5%. Conclusion The locking plate fixation of proximal humeral fractures with type II, III, is the treatment of proximal humeral fractures with the better choice. 【Key words】Proximal humerus humerus locking plate 肱骨近端骨折临床常见，占全身骨折的4%～5%，多见于老年骨质疏松患者[1～6]。对肱骨近端骨折的治疗，目前大多数学者主张采用手术内固定，内固定方法较多，我院自2008年5月～2011年6月采用锁定钢板固定肱骨近端骨折26例，效果满意，现回顾分析如下。 1 资料与方法 1.1 一般资料：26例中，男10例，女16例；年龄45～80岁，平均64岁；左侧18例，右侧8例。跌倒摔伤21例，车祸伤5例。按Neer分型：Ⅱ型19例，Ⅲ型7例。26例均为闭合性骨折。来院时间为伤后2h～4d，手术时间为伤后2～8d。合并高血压13例，糖尿病9例，慢性支气管炎6例。
