·166 · 腹部外科 2011年第24卷第 3期 FuBuWaiKe，Jun．2011，Vo1．24，No．3 CTA对小肠间质瘤的诊断价值 罗建飞 邹力 马书进 童仕伦 【摘要】 目的 评价螺旋 CT与螺旋 CT血管造影 (MSCTA)对小肠问质瘤的诊断价值 。方法 回顾性分析 19例经手术病理证实小肠间质瘤的螺旋 CT扫描与 MSCTA结果。结果 19例中良 性 12例 ，恶性7例；肿瘤大小约2～15cm。19例中CT及 MSCTA发现病灶有 16例，即定性诊断准 确率为84．21 。根据手术中探查结果，CT及MSCTA定位诊断的准确率为73．68 。小肠间质瘤 螺旋CT及MSCTA影像表现有：肿瘤呈软组织块影 ，平扫密度均匀 9例，其 中2例见坏死灶，1例见 钙化。密度不均匀 7例 ，肿块呈分叶状 ，同时伴有肠腔狭窄。2-I~Hi,肠梗阻病人有肠套叠表现。增强 扫描显示 9例呈不均匀的明显强化，7例呈均匀的明显强化，在动脉期较明显，可以看到病灶 中心坏 死，周围呈花边样强化。CT显示 1例肝转移，1例腹膜多发转移。16例MSCTA见：15例肿瘤由肠 系膜上动脉供血，1例由腹腔动脉供血，肿瘤供血血管明显增粗 ，并且可以看到明显增粗的回流静脉。 结论 螺旋CT与MSCTA无创伤性，在小肠问质瘤定性、定位诊断及指导治疗方面有着重要的临床 意义。 【关键词】 胃肠道间质肿瘤 ；小肠；体层摄影术，螺旋计算机 DiagnosticvalueofspiralCT andmulti-slicespiralCT angiography in intestinalstromaltumor LUO Jian-}et，ZOULi，MAshu_jin，eta1．DepartmentofGastrointestinalSurgery，RenminHospitalof hanUniversity， han430060， ina [Abstract] Objective ToevaluatetheimagingdiagnosticvalueofspiralCTandmulti-slicespi— ralCT angiography(MSCTA)inintestinalstromaltumor．Methods Nineteencasesofintestinalstro maltumorconfirmedsurgicallyandpathologicallywith integratedCT andpathologicdatawereana— lyzedretrospectively．Results Twelvecaseswerebenign and7casesweremalignant．Themaximum diameterofthesetumorsrangedfrom 2ClTIto15clr1．AccuracyrateofCT diagnosisfor1ocalizingand differentiatingthebenignormalignantintestinalstromaltumorwas73．68 and84．21 respective— lv．Thetumorsappearedashomogeneousdensesolid(9cases)，twocaseshadnecrosisregionand1case hadcalcification_Thetmu orsappearedasheterogenoeusdensesolid(7cases)，andtheshapewas1ob— ulatedwith intestinaltractstricture．Ninecasesappearedconspicuousenhancementand7casesap— pearedin homogenoeusehn ancement．Necrosischangesin the centerand lacepattern ehn ancement werefoundespeciallyinarterialphase．Thedilatedtmu orfeedingvesse1couldbefoundonMSCTA． Feedingvesselwassuperiormesentericarteryin15cases，andceliacarteryinonecase．Conclusion MSCTA iSsimpleandfeasibleSpiralCT examinationcandemons
