Continuity of Care Models Various models of COC have been developed and implemented in different countries. These models are classified based on the approach used to coordinate care and the setting of healthcare delivery (Kringos et al., 2010). Some of the models include: 1. Provider-centered COC: This model is centered on the healthcare provider, who acts as the primary coordinator of care. The main aim of this model is to ensure that patients receive the same level of care and treatment from different healthcare providers. 2. Patient-centered COC: This model is centered on the patient, who is seen as the primary coordinator of care.
