精品论文 参考文献 1252例小儿肠套叠的临床特点 湖南省儿童医院 湖南长沙 410007 【摘 要】目的 探讨小儿肠套的的临床特点，提高儿童肠套叠的诊治水平。方法 回顾性总结了***儿童医院2011~2012年收治的1252肠套的患儿的客观临床资料，分析其发病特点。结果 1252例肠套叠患儿中，男867人，女385人，平均年龄17.8月，中位年龄10.5月，男女比2.25：1，春夏季发病人数显著多于秋冬季。首次空气灌肠成功率89.26%，有13.24%的患儿出现肠套叠复发，部分患儿多次发作。185例患儿行手术治疗，继发性肠套叠20例（1.60%）。肠道息肉、麦克尔憩室、肠源性囊肿为最常见继发性因素。肠套叠空气灌肠初始套头部位距离回盲部越远，空气灌肠成功率越低。结论 肠套叠多发生在3岁以下婴幼儿，男性多于女性，以原发性肠套叠为主。肠套叠空气灌肠复位后患儿易出现复发，特别是在发病1周内。手术复位后患儿未见7天???复发者，且远期复发率也显著低于空气灌肠复位后患儿。肠套叠套头深度与空气灌肠复位成功率显著相关，临床上应反复跟家属反复强调“早发现、早治疗”的意义。 【关键词】儿童；肠套叠； The clinical features of 1252 pediatric intussusception cases. Abstract Objective To investigate the clinical features of children with intussusceptions，and improve the diagnosis and treatment. Methods We Retrospectively reviewed the clinical data of 1252 children with intussusception from 2011 to 2012 in Hunan Childrens Hospital，and analyzed the characteristics of the disease. Results There are a total of 1252 cases of intussusception patients，male 867，female 385 people. The average age is 17.8 months，the median age is 10.5 months，Male and Female ratio is 2.25：1.There are much number of cases in spring and summer than in autumn and winter. First air enema success rate is 89.26%，There are 13.24% of children with recurrent intussusceptions. 185 cases of children require surgery，20 cases are secondary intussusceptions，Intestinal polyp，Meckels diverticulum，intestinal cyst is the most common secondary factors. The farther Hedging site away from the ileocecal，the lower the success rate of air enema. Conclusion Intussusception occurs mostly in infants and children under 3 years old. More men than women，and mainly in primary intussusceptions. Intussusception easy to relapse，especially in the onset of 1 week. Intussusception in children rarely relapse after reduction surgery. Intussusception hedging depth significantly correlated with air enema reduction success rate，So we should emphasize with the families，early detection and early treatment， is very important.
