Diphenhydramine is a first-generation antihistamine commonly used to treat allergic reactions, but it can also induce allergic reactions. Case presentation A 32-year-old male with a history of allergic rhinitis presented to the emergency department with facial swelling, hives, and difficulty breathing. The patient reported that he had eaten shrimp for the first time in his life a few hours prior to arrival. The patient was given 50 mg of diphenhydramine intravenously for the treatment of his allergic reaction. Within a few minutes, the patient developed respiratory distress, hypotension, and shock. His blood pressure dropped to 70/40 mmHg and he became unresponsive.
