中文摘要目的：分析白介素-27（IL-27）在结核性胸膜炎中胸水的表达变化，IL-27 与腺苷脱氨酶（ADA）含 量是否存在相关性。揭示 IL-27 在结核性胸膜炎中发病机制的作用以及在结核性胸膜炎诊断中的应 用价值。方法：选择 2011 年 12 月至 2012 年 9 月在石河子大学医学院第一附属医院就诊的胸腔积液患者，经 过仔细询问病史，体格检查及必要的相关辅助检查，最终诊断为结核性胸膜炎患者 38 例纳为实验组， 同时收集癌性胸腔积液患者 30 例纳为对照组。收集两组实验对象的胸腔积液标本。采用酶联免疫吸 附法（ELISA 法）测定患者胸腔积液中 IL-27 的含量。采用全自动生化分析仪测定 ADA 水平含量， 由检验科专业技师协助检测。使用 SPSS17.0 统计软件包进行统计分析，计量资料结果以均数士标准差(±s)表示；对于正态分布的方差齐的变量，采取 t 检验进行两组间比较；非正态分布的变量经对 数转化后采用秩和检验。IL-27 与 ADA 的相关性分析采用 Spearman 相关分析。 P＜0.05 为差异具有统计学意义。结果：结核性胸膜炎组胸腔积液中检测到的 IL-27 水平含量，比在癌性胸腔积液组中检测的 IL-27 水 平显著增高（Z=-7.04，p0.05，差异具有统计学意义）。两组受试者 ADA 水平含量比较，结核性 胸膜炎组 ADA 水平含量显著高于恶性胸腔积液组（ Z=-7.028，p0.05，差异具有统计学意义）。 确定 IL-27 在诊断结核性胸膜炎中的最佳诊断临界值为 167.02ng/l，灵敏度和特异度分别为：94.7% 和 86.7% 。胸腔积液中 IL-27 含量与 ADA 含量呈显著的正相关关系（r=0.0727p＜0.01）。 结论：IL-27 可能参与了结核性胸膜炎的炎症反应，对结核性胸膜炎的鉴别诊断中有一定的价值。与 ADA 联合检测可作为临床诊断结核性胸腔积液的有效炎性标记物。关键词：结核；胸膜炎；IL-27；诊断AbstractObjective: To investigate the IL-27 of the tuberculous pleurisy pleural effusion expression changes, IL-27 and adenosine deaminase (ADA) content whether there is a correlation.
