正常对照组，但差异无统计学意义(P 0.05)。2）IL-1B-511、IL-1B-31、IL-1B+3954和IL-1RN基因型符合Hardy-Weinberg遗传平衡，具有群体代表性。3）IL-1B-511、-31、+3954各组基因频率及基因型，在十二指肠溃疡和胃溃疡组与正常对照组之间比较均无显著性差异（P0.05）。IL-1B同时含有-511*C、-31*T 和+3954*C等位基因时，在十二指肠溃疡和胃溃疡组与正常对照组之间比较也无统计学差异（P0.05）。4）十二指肠溃疡组IL-1RN*2基因频率明显高于正常对照组(19.3% vs 9.8%， P=0.046），IL-1RN L/2和2/2基因型的OR为2.31，95%CI为1.07~4.98。在H.pylori感染阳性人群中，十二指肠溃疡组IL-1RN*2基因频率与正常对照组之间比较，差异有统计学意义（ P=0.024 ），其含 *2 等位基因的 L/2 和 2/2 基因型的 OR 为 3.29（95%CI=1.22~8.83）。IL-1RN各基因频率及基因型在胃溃疡组和正常对照组之间比较无统计学差异（P0.05）。5）logistic回归调整年龄、性别和吸烟混杂因素后，H.pylori感染和含*2等位基因的 L/2 和 2/2 基因型对十二指肠溃疡发生的 OR 值分别为 18.01 和 3.97.（95%CI=4.70~63.67和1.53~10.34）。6）治疗2周后对消化性溃疡患者进行胃镜检查，IL-1B-511、-31、+3954和IL-1RN 各组基因频率及基因型，在十二指肠溃疡组和胃溃疡组痊愈者与未痊愈者之间比较均无统计学差异（P0.05）。IL-1B同时含-511*C、-31*T、+3954*C等位基因时，十二指肠溃疡组和胃溃疡组痊愈者与未痊愈者之间比较也无统计学差异（P0.05）。结论1) IL-1RN*2等位基因增加个体患十二指肠溃疡的风险。2) H.pylori和IL-1RN*2是十二指肠溃疡发生的危险因素,二者发挥着协同的作用。3) IL-1B和IL-1RN基因多态性与胃溃疡的发病可能无关。4) IL-1B和IL-1RN基因多态性对埃索美拉唑治疗消化性溃疡的早期愈合无影响。关键词：消化性溃疡；幽门螺旋杆菌；白细胞介素-1；基因多态性Influence of the IL-1B and IL-1RN genes polymorphisms on the peptic ulcer diseaseAbstractBackground and ObjectiveHelicobacter pylori(H.pylori)is a major environmental fators involved in the pathogenesis of peptic ulcer，but only a small proportion of infected individuals develop the disease.the underlying pathogenic mechanisms have not been fully understood，Generally considered that the interactions between environmenta,bacterial and multiple genetic components are likely to be involved.IL-1B and IL-1RN encode IL-1β and IL-1ra,respectively. IL-1β is a pro-inflammatory cytokine,and is the powerful inhibitor of gastric acid, IL-1ra is its natural antagonist.
