摘要老年冠心病患者抑郁与肿瘤坏死因子的相关研究研究生：刘宇导师：古丽扎尔 副教授摘要目的：初步探讨老年冠心病患者肿瘤坏死因子（TNF）的改变及抑郁对 TNF 的 影响。方法：连续收集 2008 年 8 月至 2008 年 11 月住院的老年稳定性心绞痛患者214 人。根据中国精神疾病分类与诊断标准第三版（CCMD-3），将纳入患者分为冠心病合并抑郁组 54 例，冠心病不伴抑郁组 160 人，从新疆医科大学体检的老年人中 随机选择 50 人作为健康对照组。测量各生化指标，ZUNG 抑郁自评量表得分和 TNF 值，观察各组 TNF 的差异及 TNF 与抑郁得分的相关性。结果：老年冠心病患者抑 郁症的检出率为 25.23%。健康对照组.冠心病组.冠心病合并抑郁组 TNF 水平有差异， 三组间差异有统计学意义（P＜0.05）；冠心病合并抑郁症组 TNF 值高于冠心病组（P＜0.05），冠心病组 TNF 值高于健康对照组（P＜0.05）；TNF 与抑郁得分呈正相关（r=0.424，P＜0.01）。控制白细胞计数与 C-反应蛋白后，TNF 仍与抑郁得分呈正相 关（r=0.344，P=0.000）。结论：老年冠心病患者抑郁症的检出率高，合并抑郁的老 年冠心病患者肿瘤坏死因子水平增高；增加 TNF 的分泌，可能是抑郁导致冠心病的 机制之一。关键词： 冠心病； 抑郁症； 肿瘤坏死因子新疆医科大学医学硕士学位论文Correlation study between tumor necrosis factor-α and depression for the elderly patients with Coronary Heart DiseasePostgraduate: Liu YuSuperviso: Prof. Gu Li ZaerAbstractObjective: To investigate the changes of tumor necrosis factor alpha (TNF-α) in elderly patients with coronary heart disease and the impact of depression on the TNF-α.
