福建医科大学硕士研究生毕业论文 福建医科大学硕士研究生毕业论文 福建医科大学硕士研究生毕业论文 福建医科大学硕士研究生毕业论文 PAGE PAGE 4 PAGE PAGE 5 Influence of CYP2C9 *3, VKORC1-1639 GA and CYP4F2 rs2108622 Genetic Polymorphisms on Warfarin Dose Abstract [Objective] To investigate the influence of the cytochrome P450, family 2, subfamily C, polypeptide 9(CYP2C9 *3), vitamin K epoxide reductase complex, subunit 1(VKORC1-1639 GA), and cytochrome P450, family 4, subfamilyF, polypeptide 2(CYP4F2 rs2108622) genetic polymorphisms on warfarin dose in patients with heart valve replacement, search for reasons of individual differences on warfarin dose, guide reasonable usage of warfarin. [Methods] Collected the patients, blood samples who were operated in the Department of Cardiovascular Surgery of Fujian Provincial Hospital from November 2011 to August 2012. Using the method of Polymerase Chain Reaction(PCR)-gene sequencing to detected 133 cases after valve surgery patients of long-term oral warfarin CYP2C9 *3, VKORC1-1639 G A and CYP4F2 rs2108622 genetic polymorphism, grouped according to genotype, compared the average dose of warfarin of different genotypes and discussed the effect of gene mutation on warfarin dose. [Results] The frequencies of CYP2C9 * 3 AA, AC, CC were 127 cases, 6 cases, 0 case, the average dose of warfarin were 3.75 mg/day, 2.13 mg/day, the difference was statistically significant (P 0.05). The frequencies of VKORC1-1639 G A GG, GA, AA were 3 cases, 32 cases, 98 cases, the average dose of warfarin were 6.0 mg/day and 4.5 mg/day, 3.0 mg/day, the difference was statistically significant (P 0.05). The frequencies of CYP4F2 rs2108622 CC, CT, TT were 67 cases, 59 cases, 7 cases, the average warfarin dose were 3.0 mg/day, 3.75 mg/day, 4.5 mg/day, the difference was statistically significant (P 0.05). [Conclusion] CYP2C9 *3, VKORC1-1639 GA and CYP4F2 rs2108622 genetic polymorphisms were associated with the difference of individual warfarin dose, and then established the multiple linear regression model: Y=5.36
