精品论文 参考文献 心理干预在无创呼吸机治疗COPD伴Ⅱ型呼衰患者中的护理探讨 （四川省自贡市第一人民医院呼吸科 四川 自贡 643000） 【摘 要】目的：探讨心理干预在COPD伴Ⅱ型呼衰患者中使用无创呼吸机辅助通气过程中的作用及效果。方法：搜集我科2013年1月-2014年12月入院治疗的COPD伴有Ⅱ型呼衰的62名患者，随机分为对照组与干预组（每组31人）。对照组按照COPD进行常规护理及无创呼吸机辅助治疗（BiPAP），干预组在常规护理及无创呼吸机辅助治疗基础上，在治疗前、治疗期间，尤其是第1小时、6小时，医生、护士对病人进行心理干预。分别在使用无创呼吸机前、后检测血气分析，通过对PaCO2 、PaO2指标水平的变化情况来判断心理干预的疗效。结果：干预组患者较对照组患者的PaCO2 有明显下降，PaO2 有明显升高，差异具有统计学意义（plt;0.05）。结论：心理干预可有效减低患者对呼吸机的恐惧心理，能够加快患者适应呼吸机，对疾病的预后起到积极作用。 【关键词】无创呼吸机（BiPAP）；COPD；Ⅱ型呼吸衰竭；心理干预 【中图分类号】R47 【文献标识码】B 【文章编号】1003-5028（2015）6-0784-02 The research of psychological intervention in the treatment of noninvasive mechanical ventilation for COPD patients with type Ⅱ respiratory failure 【Abstract】Objective: To investigate the role and effects of psychological intervention in the treatment of noninvasive mechanical ventilation for COPD patients with type Ⅱ respiratory failure .Methods: 62 COPD patients with type Ⅱ respiratory failure from January 2013 to December 2014 in our hospital.patients were randomly divided into a control group and the intervention group (n = 31). Control group accepted routine care and non-invasive ventilator-assisted therapy (BiPAP), the intervention group besides routine care and non-invasive ventilation (BiPAP), accepted psychological intervention from doctors nurses before and during the treatment period,especially the first and sixth hour. Blood gas analysis was tested before and after using non-invasive ventilator, through the changes of PaCO2, PaO2 index level to determine the efficacy of psychological interventions. Results: The level of PaCO2 was more decreased and PaO2 was more increased in the intervention group than the control group, the difference was statistically significant (p lt;0.05). Conclusion: Psychological interventions can effectively reduce the patients fear to the ventilator, the patient can adapt ventilator earlier.
