E-mail:zhuqtgz@ Mobile:[Abstract] Objective To summarize the clinical features of blunt major arterial injury in the extremity so as to provide the basic evidences for developing specific management strategy. Methods The patients having major arterial injury in the extremity admitted to the First Affiliated Hospital of Sun Yat-sen University from January, 1998 to June, 2009 were included in this study. Based on the mechanism of injury, they were divided into Blunt Injury Group (BIG) and Penetrating Injury Group(PIG). The charts were reviewed and the information regarding demographics, clinical course and early outcomes was collected and analyzed. Result There were 61 cases in BIG. The mechanism of injury was dominant at road traffic injury(34 cases) and crush injury (22 cases), accounting for 91.80%. The lower limb injury accounted for 73.77%. The blunt major arterial injuries were usually associated with fractures, hemorrhagic shock and multiply injuries, accounting for 98.36%, 24.59% and 11.48% respectively. Vascular grafting was performed in 45.65% of total cases who underwent vascular reconstruction. The overall amputation rate was up to 49.18%. While the patients in PIG were mainly injured by cut, the most common injury site was the upper limb, without severe bone and soft tissue injury. Treatment was relative simple with few complications.
