IL-18、NF-κB、iNOS在早期自然流产患者中的表达中文摘要目的：检测早期自然流产妇女、正常早期妊娠妇女外周血、绒毛及蜕膜中白细胞介素－18(Interleukin-18，IL－18)、核转录因子(Nuclearfactor-kappaB，NF－?B)及诱导型一氧化氮合酶(Induciblenitricoxidesynthase，iNOS)的表达与分布情况，探讨IL－18、iNOS和NF－?B的表达与早期自然流产的关系。方法：根据诊断标准入选，从2011年06月至2012年06月收集于福建医科大学附属第一医院妇科门诊就诊符合诊断标准的病例31例为病例组及同期于门诊行人工流产手术的30例为对照组。采用免疫组织化学SP法染色检测两组绒毛及蜕膜中IL－18、NF－?B、iNOS的表达情况，用ELISA和硝酸酶还原法检测血清中IL－18和iNOS的含量。结果：1.一般临床资料比较：病例组和对照组的年龄无统计学差异(p0.05)；病例组和对照组的停经天数无统计学差异(p0.05)。2.IL-18、NF-?B和iNOS在病例组和对照组的绒毛及蜕膜中均有表达。IL－18主要表达于细胞胞浆，阳性颗粒呈浅黄色到深褐色。NF－?B主要表达于细胞浆和细胞核，阳性染色呈浅黄色到深褐色。iNOS为胞浆染色，阳性染色呈浅黄色到深褐色。与对照组相比，病例组绒毛中IL－18、NF－κB和iNOS呈过度表达(P0.05)，蜕膜中iNOS呈过度表达(P0.05)。3.外周血中IL－18、iNOS的含量病例组明显高于对照组(P0.05)。4.在病例组中IL－18的表达与NF－?B的表达呈正相关(r=0.392，p=0.029)；NF-?B的表达与iNOS的异常表达呈正相关(r=0.563，p=0.001)。结论：1、IL－18、NF－?B和iNOS在病例组绒毛及血清中的表达明显高于对照组。2、IL－18、NF－?B和iNOS与早期自然流产有关。3、异常表达的IL－18、NF－?B和iNOS使1型辅助T细胞（Th1）和2型辅助T细胞（Th2）系统（Th1/Th2）平衡破坏，找出针对三者的抑制剂来辅助治疗减少早期流产发生。关键词：早期自然流产白细胞介素－18核因子－κB诱导型一氧化氮合酶ExpressionofInterleukin－18、Nuclearfactor－kappaB、InduciblenitricoxidesynthaseinearlyspontaneousabortionAbstractObjective:TodetectthedistributionandexpressionofInterleukin-18（IL-18）,Nuclearfactor-kappaB（NF-κB）andInduciblenitricoxidesynthase（iNOS）ofperipheralblood、villiandduciduainearlyspontaneousabortion,normalearlypregnancy,toexplorethepossiblerelationbetweenthesefactorsandearlyspontaneousabortion.Method：Accordingtothediagnosticstandard,FromJun2011toJun2012,Tocollect31caseswhometthediagnosticcriteriaand30normalwomenwhoareperformedartificialabortionvoluntaryascontrolgroupofthesameperiodinmyout-patientclinicofFirstAffiliatedHospitalofFujianMedicalUniversity,TheexpressionofIL-18,NF-?BandiNOSofvillianddeciduaweredetectedusingimmunohistochemistrystaining(S-P),ThelevelsofIL-18andiNOSinthesupernatantweredetectedbyenzyme-linkedimmunosorbentassay(ELISA)andthenitricacidreductasemethodofthetwogroups.Results:1、Generalclinicaldatacomparison:Comparedwithnormalearlypregnancygroup,therewerenosignificantlydifferenceinaverageageandaveragemenopausedays.2、IL-18，N
