Survivin与Ki-67在结直肠腺瘤和腺癌中表达的临床意义 　　【摘要】 目的 研究存活素(Survivin)在结直肠腺瘤和腺癌中的表达及其临床意义。方法 应用免疫组织化学SABC法检测139例结直肠腺瘤及56例结直肠腺癌中Survivin及Ki-67的表达情况,分析二者与各临床病理指标之间的关系。结果 Survivin与Ki-67在结直肠腺瘤中的表达水平与性别、年龄及病理类型无关;而与腺瘤上皮内瘤变的级别有关。Survivin与Ki-67在结直肠腺癌中的表达水平与性别及年龄无关;而与Dukes分期、分化程度及淋巴结转移情况有关。Survivin与Ki-67在腺癌中的表达水平明显高于在腺瘤中的表达,差异具有统计学意义(χ2值分别为30.014、23.142,P值均2值分别为0.129、0.007,P值分别为0.720、0.0930.05)。在结直肠腺瘤及腺癌中Survivin与Ki-67的表达之间均具有正相关性。结论 联合检测Survivin及Ki-67有助于判断结直肠腺瘤及腺癌的恶性趋势及生物学行为,为结直肠腺瘤及腺癌的个体化治疗提供依据。 　　【关键词】 上皮内瘤变;腺癌;存活素;细胞核增殖相关抗原 　　 　　Clinical significance of survivin and Ki-67 in Colorectal Adenoma and Adenocarcinoma 　　 　　ZHI Qing-jiang,LI Shu-liang,XIE Jian-jun.Department general surgery,the Second People’s Hospital of Liaocheng,Linqing Shandong,Liaocheng 252600,China 　　【Abstract】 Objective Investigate the expression of Survivin in colorectal adenoma and adenocarcinoma,and then analyze their correlation and their relationships with clinico-pathological features in order to explore the effect of Survivin in the transition from adenoma to adenocarcinoma.Methods Immunohistochemical(IHC)was used to detect the expressive levels of Survivin and Ki-67 in specimens collected from 139 patients with colorectal adenoma and 56 patients with colorectal adenocacinomal,to carry out statistic analysis compared with clinical features.Results There is correlation between Survivin,Ki-67’s expressive levels in colorectal adenoma and the grade of intraepithelial neoplasia,but no correlation between their expressive levels and gender,age as well as pathological typing.There are correlations between Survivin,Ki-67’s expressive levels in colorectal adenocarcinoma and Dukes stage,pathological grading as well as lymphatic metastasis,but no correlation between their levels and gender as well as age.Both the Survivin and Ki-67’s expressive levels in adenocarcima are manifestly higher than in adenoma,the disparation has statistical significance(χ2=30.014,23.142,P=0.0002=0.129,0.007,P=0.720,0.0930.05).There a
