精品论文 参考文献 逆向切除联合羊膜移植＋角膜缘干细胞移植治疗 黎燕英 吴艺 戎志銮 （广东省第二人民医院 眼科广州510317） 【摘要】目的：探讨逆向切除联合羊膜移植＋角膜缘干细胞移植治疗翼状胬肉的临床效果和可行性。方法:选取76例82眼翼状胬肉患者，显微镜下行翼状胬肉逆向切除联合羊膜移植＋角膜缘干细胞移植术，观察术后角膜愈合情况，羊膜移植片存活、胬肉复发率等指标。结果:76例82眼复发4眼，复发率为4.8％，术后不适症状轻，持续时间短，角膜创面修复时间短。结论 逆向切除联合羊膜移植＋角膜缘干细胞移植治疗翼状胬肉复发率低，角膜创面修复快，有效阻止异常组织及新生血管长入角膜，是一种较理想的术式。 【关键词】逆向切除;羊膜;角膜缘干细胞;移植;翼状胬肉 【中图分类号】R779.6【文献标识码】A【文章编号】1008-6455（2010）08-0043-02Clinical observation of fresh amniotic membrane transplantation combined with auto-corneal limbus stem cell transplantation in the treatment of improved pteryglum surgery Li YanyingWu YiRong ZhiLuan 【Abstract】Objective:Aim To investigate the clinical effet and feasibility of fresh amniotic membrane transplantation combined with outo-corneal limbus stem cell transplantation in the treatment of improved pteryglum surgery. Methods:Seventy six pterygium cases (82 eyes) underwent improved pterygium excision operation, the eyes were given fresh amniotic membrane transplantation combined with outo-corneal limbus stem cell transplantation. Results:All the cases were followed up for 6~18 monthes. The ptergium recurrence appeared in four eyes, and the recurrence rate was 4.8%. Pterygium excision causedless discomfort, took less time and helped wound surface to heal quickly. Conclusion:These findings suggest that in the plergium inproved surgey fresh amniotic membrane transplantation combined with outo-corneal limbus stem cell transplantation can significantiy decrease the recurrence rate of pterygium, and wound surface heal quickly, and it can stop the abnormal tissue and new vessels growing into the cornea. So we think it is an effecfive method. 【Key words】Pterygium;Transplantation;Auto-corneal limbus stem cell;Fresh amniotic membrane;Improved surgery 翼状胬肉是眼科的常见病、多发病，轻者影响外观，重者引起角膜性散光、角膜变性，遮盖瞳孔等致视力下降。其确切病因不明确，治疗以手术为主。传统的手术方法复发率30％～50％[1]，甚至有资料报导可高达69％[2]。为降低翼状胬肉术后复发，在单纯切除胬肉的基础上，联合结膜瓣转移、羊膜移植，带角膜缘干细胞移植，联合应用丝裂霉素等改良术式，均有效降低了翼状胬肉术后复发率。2005年1月～2008年1月我们采用逆向切除胬肉联合羊膜移植＋角膜缘干细胞移植术，明显降低了术后复发率。现报
