精品论文 参考文献 胆源性胰腺炎手术干预时机的临床分析 浏阳市官渡中心医院外科 湖南浏阳 410304 【摘 要】 目的 探讨不同手术干预实际对胆源性胰腺炎手术治疗效果的影响。方法 收集2013 年10 月至2015 年10月我院收治的胆源性胰腺炎手术患者90 例，对其临床资料进行回顾性分析，其中45 例患者行早期手术干预，作为对照组，45 例患者行延期手术干预，作为观察组，比较两组患者APACHEⅡ评分、住院时间、并发症和死亡率。结果 轻症胰腺炎患者和重症胰腺炎患者中，观察组患者的APACHEⅡ评分、住院时间、并发症评分均明显优于对照组，Plt;0.05；重症梗阻性患者中，对照组患者的死亡率明显低于观察组，Plt;0.05。结论 手术治疗时机对于胆源性胰腺炎患者的治疗效果存在显著影响，对于轻症胰腺炎和重症非梗阻胰腺炎患者，早期宜行保守治疗，依据患者实际情况选择合适时机延期行手术治疗，可有效提高患者治疗效果，降低并发症，对于重症梗阻性胰腺炎患者，早期行手术干预治疗可降低死亡率，但是并发症较多，临床应用中应权衡利弊，选择适宜时机进行治疗 【关键词】不同手术干预；胆源性胰腺炎；手术治疗 【Abstract 】 objective to investigate different surgical intervention of the actual effect of surgical treatment of gallstone pancreatitis.Collected in October 2013 to October 2015，our hospital 90 cases of patients with gallstone pancreatitis surgical，the clinical data were retrospectively analyzed，including 45 cases of patients with early surgical intervention，as control group，45 patients underwent surgical intervention，delay as observation group，compared two groups of patients with APACHE Ⅱ grade，length of hospital stay，complications and mortality.Results mild pancreatitis patients and in patients with severe acute pancreatitis，observation group of patients with APACHE Ⅱ grade，length of hospital stay，complications were significantly better than the control group，P lt; 0.05; In patients with severe obstructive，the patients mortality is significantly lower than the observation group，the control group（P lt; 0.05）.Conclusion surgery timing for the treatment of patients with gallstone pancreatitis effect exists significant influence，for mild pancreatitis and severe obstruction in patients with acute pancreatitis，early appropriate line of conservative treatment，according to the actual condition choose appropriate time delay in patients with surgical treatment，can effectively improve patient treatment effect，reduce the complications，patients with severe obstructive pancreatitis，early surgical intervention can decrease the mortality，but more complications，
