精品论文 参考文献 多层螺旋CT在尿路梗阻病变中的应用价值 李月梅 (辽宁省沈阳市第五人民医院 110011) 　　【摘要】目的 探讨多层螺旋CT及其后处理技术对诊断尿路梗阻疾病的诊断价值。方法 收集临床怀疑有泌尿系统梗阻性疾病，行16层螺旋CT扫描后经过多平面重建（MPR）、曲面重建（CPR）及容积重建（VR）等后处理及进行尿路成像，并经手术及临床证实的32例病例，并对结果进行了研究。结果 泌尿系结石21例，输尿管肿瘤5例，输尿管结核2例，先天畸形4例，多层螺旋CT诊断在定性及定位诊断上符合率明显高于腹部平片（KUB）、B超及静脉肾盂造影（IVP）检查。结论 多层螺旋CT泌尿系成像检查是一种无创检查，操作简单，比以往传统的影像学检查方法的诊断更符合手术的结果，说明多层螺旋CT在诊断泌尿系结石、肿瘤及其他梗阻性疾病的临床价值上是有独特的优势的。 　　【关键词】多层螺旋CT 后处理技术 泌尿系梗阻病因诊断 　　【中图分类号】R445 【文献标识码】A 【文章编号】1672-5085（2013）41-0128-02 　　【Abstract】Objective To explore the diagnostic value of multi slice spiral CT and post-processing technique in urinary tract obstruction diseases.Methods clinical suspected urinary tract obstruction, 16 slice spiral CT scanning after MPR、CPR and VR treatment and urinary tract imaging, and 32 cases were confirmed by operation and clinic, and the results were studied. Results of 21 cases of urinary calculi, 5 cases of ureteral cancer, 2 cases of tuberculosis of ureter, congenital malformation in 4 cases,multi-slice spiral CT in diagnosis of qualitative diagnosis and localization coincidence rate was significantly higher than that on the plain abdominal radiograph (KUB), B ultrasound and intravenous pyelography (IVP)examination. Conclusion multi slice spiral CT urography examination is a noninvasive, simple operation, image diagnosis than the traditional examination method is more in line with the operation results, illustrate the clinical value of multi-slice spiral CT in the stones, tumors and other obstructive disease diagnosis of urinary system is a unique advantage. 　　【Key words】multi-slice spiral CT post processing technique urinary obstruction diagnosis 　　1 资料与方法 　　1.1一般资料 　　收集经手术及临床证实的32例的泌尿系梗阻患者作为研究对象，这些患者在住院期间同时进行了B超、IVP检查及多层螺旋CT平扫及三维重建；其中男性21例，女11例，年龄26-76岁，平均51岁；多数病例的症状为腰部不适、腰痛、尿频、尿急，血尿等。 　　1.2研究方法 　　患者空腹12h、喝水憋尿后行GE16层螺旋CT扫描，扫描范围自肾上极至耻骨联合。平扫的层距为5mm，并进行1.25mm的薄层重建，以备在工作站进一步行三维后处理。部分行多层螺旋CT的增强扫描，扫描层厚为5mm，重建层厚为1.25mm，用高压注射器以2.5ml/s的流速注入造影
