冠心病患者 PCI 术前后血清 YKL-40 及 hs-CRP的变化及相关性研究 摘要目的:通过测定冠心病患者介入治疗前后血清 YKL-40 和 hs-CRP 浓度的变化，探究 两者的变化规律及它们之间相关性；方法:入选山医大一院行择期经皮冠脉介入病例 76 例，分析各例临床资料将其分为 稳定心绞痛组(SAP)26 例，不稳定心绞痛组(UAP)24 例，急性 ST 段抬高或非 ST 段抬高的心梗组(STEMI 或 NSTEMI)26 例，同期纳入经冠造（CAG）证实无冠脉 病变的对照病例 22 例。采用酶联免疫吸附法检测入选病例行冠脉造影或介入治疗 术前后的 YKL-40 及 hs-CRP 水平，并分析其相关性。结果:(1).与冠脉造影未见明显斑块正常参照组对比，SAP 组、UAP 组、心梗组血清 YKL-40 及 hs-CRP 水平均有明显增高，且有依次递增趋势，与正常对照组较差异具 有统计学显著性（p0.05）；(2).除正常参照组外，余各组冠脉介入治疗术后血清 YKL-40 和 hs-CRP 浓度较术前增高,差异具有显著性（p0.05）；(3).PCI 术前后血 清 hs-CRP 浓度与 YKL-40 浓度呈正相关，相关系数分别为 0.649 和 0.654。结论:(1).YKL-40 联合 hs-CRP 可能更好的评估冠心病患者炎症状态，一定程度上反 应冠脉斑块的不稳定性；(2).YKL-40 可能在评估术后支架的再狭窄及主要不良心血管事件有一定的价值。关键词: 冠心病；择期冠脉介入治疗；超敏 C 反应蛋白；人类软骨糖蛋白 39Research about the Changes and Correlations of serum YKL-40 and hs-CRP in CAD patients with PCIAbstractObjective:Explore variation and correlation by measuring serum YKL-40 and hs-CRP concentrations in patients with coronary heart disease before and after interventional treatment .Methods:SelectedTHEFIRSTAFFILIATEDHOSPITALSHANXIMEDICALUNIVERSITY cases, 76 cases of primary percutaneous coronary intervention (PCI), according to its clinical Symptom of divided into stable angina pectoris (SAP) group 26 cases, 24 cases of unstable angina pectoris (UAP) group and acute myocardial infarction (AMI) group 26 cases, at the same time selected coronary angiography confirmed without coronary artery lesions in 22 cases of normal control group.
