摘要摘要 摘要 摘要 厦门市以调整和优化医疗卫生资源结构为手段，加快构建新型城市卫生服务 二级体系，2008年确立了政府主导、体现公益性质的社区卫生服务“厦门模式一。 这一模式解决了区域卫生存在的很多问题，在全国产生较大影响。如何建立一套 可推广可移植的区域协同医疗集成平台和运行机制，为创建全新的符合国家新医 改政策的现代医疗服务模式提供强大的信息化技术支撑能力，把三级医院和社区 形成一个整体，．使总院与社区实现真正意义上的一体化，同质化，初步解决“看 病难’’、“看病贵、“看病乱是一迫切的问题。 本文在分析了建立符合“厦门模式’’的社区医疗服务中心信息系统重大意义 的基础上，详细阐述了社区医疗服务信息系统的需求分析、设计和实现。该系统 可有效实现病患完整的诊疗信息在社区与医院之间的共享与交换，促进居民就医 模式向“小病在社区，大病到医院，康复回社区，保健不出门的转变，从而极 大的优化医疗资源的运行负载，推动医疗卫生行业的健康发展，为人民谋取更大 利益。 关键词：区域医疗；厦门模式；HIS Abstract‘Xiamen Abstract ‘Xiamen mode’for the community health service was established in 2008 which was government-led and reflecting the public nature through the adjusting and optimizing of the structure of health ca∞resources and also the accelerating the construction of secondary city health service system．This mode solves many of the existing regional health problems and has a greater impact in the whole country．It’S an urgent issue about how to setup a portable regional collaboration health care integration platform and its running system．It is to provide a powerful information technical support for the new modem health care models which is under construction and suiting for the health care reform policy．And it’S also to form the tertiary hospital and community hospital as a unity,integrate and homogenize the general hospital and its community hospitals．Wim this system it’S to imtially solve the problems such as ‘hard to see doctor’and“expensive to see doctor’，and SO on． This dissertation introduces the significance of the construction of community health service information system．On this basis，the dissertation proposed the system requirement analysis，system design and its implementation．This Syst锄will lead to the effective interventions on treatment of patient information between hospitals and communities．This will promote the model change for people to optimize the health care resources，and promote the healthy development to seek greater benefits for the people． Keywords：Regional Health；Xiamen Mode；HIS 第一章绪论第一章 第
