II II 中文摘要梗阻性黄疸患者术后早期肠内营养的临床研究【结论】：梗阻性黄疸患者术后施行早期肠内营养改善了病人的营养代谢状态，且安全有效；可有效保护肠粘膜屏障(IBF)，对预防发生术后肠道细菌/内毒素（BET）移位及感染并发症有着重要意义；可促进胃肠功能的恢复，缩短住院时间及降低住院费用。【关键词】：梗阻性黄疸，肠内营养，肠外营养，肠粘膜屏障作者：熊强指导导师：赵登秋 PAGE IV PAGE IV 梗阻性黄疸患者术后早期肠内营养的临床研究英文摘要 Clinical study of early enteral nutrition in patients with obstructive jaundice after operation Abstract Objective:By grouping the patients with obstructive jaundice after operation, and treated respectively with early enteral nutrition (EEN) and total parenteral nutrition (TPN), to observe the two kinds of nutrition on the prognosis in patients with obstructive jaundiceafter operation,to investigate the clinical effect and significance of early enteral nutrition after obstructive jaundice operation. Methods:40 cases of the patients with obstructive jaundice after operation were randomly divided into EN group and PN group in our hospital between February 2012 to February 2014.EN group was treated with early enteral nutrition, and PN group with total parenteral nutrition, 20 patients in each group.Two groups of patients with equal calorie and nitrogen of nutritional support for 7 days.The specimen of blood were obtained befor operation and on 1st、3rd and 7th day after operation to detect the level of serum albumin (ALB), prealbumin (PA), transferrin (Tf), alanine aminotransferase (ALT), aspartate aminotransferase (AST), alkaline phosphatase (ALP), γ-gamma glutamyl transferase (GGT), the plasma endotoxin and D- lactate.Calculating the daily nitrogen balance and accumulative total nitrogen balance after operation.Record the numbers of cases of postoperative complications, intestinal function recovery time, treatment of the associated costs and postoperative hospitalization days. Results:The differences of the level of serum prealbumin, albumin and transferrin were not statistically significant in the two geoup whether between or within group comparisons(P0.05).
