Peutz―Jeghers综合征3例报告并文献复习 　　[摘要] 目的 探讨黑斑息肉病的临床特征、诊断及治疗，提高对Peutz-Jeghers综合征的认识，为临床诊疗提供参考依据。方法 选取该院2014―2016年所收治P-J综合征患者3例，回顾性分析其临床资料，包括病因、个人史、住院治疗史、相关检查、病理结果及诊断等情况。结果 3例均发现特殊部位黑色素斑，胃肠镜检查3例均发现胃、大肠息肉，1例胶囊内镜发现小肠息肉，1例术中发现小肠息肉；1例有家族遗传现象；病理均存在错构瘤，1例合并腺瘤形成；1例单纯经内镜下治疗，1例内镜下治疗后因息肉反复发生且体积大、数量多，行外科手术治疗，1例因发生肠套叠行外科手术治疗。 结论 根据P-J综合征患者特殊部位黑色素斑沉积情况，同时，结合内镜检查及病理活检，可有效提升诊断准确性；临床治疗可给予内镜下治疗，但由于内镜治疗后复发几率高，且鉴于P-J综合征与恶性肿瘤密切相关，???期随访是目前最好的防治手段。 中国论文网 /6/viewhtm 　　[关键词] P-J综合征；黑色素斑沉积；息肉 　　[中图分类号] R5 [文献标识码] A [文章编号] 1674-0742（2018）02（b）-0049-03 　　[Abstract] Objective This paper tries to explore the clinical features， diagnosis and treatment of polyposis， to improve the understanding of Peutz-Jeghers syndrome（P-J syndrome）， and to provide reference for its clinical diagnosis and treatment. Methods 3 cases of P-J syndrome treated in this hospital from 2014 to 2016 were selected， and their clinical data were retrospectively analyzed， including the etiology， personal history， hospitalization history， related examinations， pathological findings and diagnosis. Results Melanin spots were discovered in 3 cases at special parts， and gastric or intestinal polyps were found in 3 cases with the help of Gastrointestinaloscopy. The small intestinal polyp was found in 1 case by capsule endoscope， while another was detected in surgery. 1 case of family genetic phenomena. Furthermore，1 case was resulted from the combination with adenoma. 1 case was only with endoscopic treatment， and while the other case was treated by endoscope， and surgical treatment was given due to the recurrence， the big size and the large amount of the polyps after the initial treatment. Conclusion Full understanding of the situation of deposition of melanin spots appearing in the P-J syndrome patients special parts， and combination of endoscopy and biopsy can contribute to effectively improving the diagnostic accuracy.
