University of California, San Francisco School of Nursing Faculty Practice Policy and Procedures FACULTY PRACTICE HEALTH PERSONNEL NEW HIRE PROCESS I. PURPOSE A. To delineate a consistent approach for the hiring of salaried and volunteer faculty and staff into one or more of the School of Nursing Faculty Practices (FP). POLICY The hiring of FP faculty and staff (salaried and volunteer) is the responsibility of FP Manager/Clinical Director and the Department Chair. The FP Manager/Clinical Director and the Department Chair will assure that all relevant certification and credentialing information has been entered into the School of Nursing centralized computer database. All faculty practice related documents for the newly hired faculty or staff will be included in the School of Nursing “Academic Personnel Packet” or the “Staff Appointment Packet,” which is available from the designated administrative staff member in each department. Step 1 of the new hire process must be completed before participating in any FP-related activities. All advanced practice nurses (clinical nurse specialists, certified nurse midwives and nurse practitioners) and physicians who will be involved in direct patient care activities must also complete step 3 before participating in FP-related activities. All advanced practice nurses, licensed vocational nurses, physicians and registered nurses must also complete step 4 before participating in FP-related activities. All faculty and staff who will have direct patient contact must complete Step 2 requirements within 30 days after the start date in the FP(s). If this step is not completed within this timeframe, the individual will not be allowed continued involvement in the FP(s). If the faculty member and staff are solely involved in a School of Nursing FP wherein no actual contact with a patient population occurs (e.g. Tele-healthcare), steps 2, 3 and 4 of the new hire process may be disregarded.
