益肾健脾按摩法早期干预对早产儿心身发育的临床研究 广州中医药大学附属南海妇产儿童医院 刘振寰 黄燕春 【中文摘要】目的：对早产儿进行远期随访发现，其神经系统损伤后遗症较足月儿高；且在婴儿期及学龄期的体格发育均较正常足月儿明显迟缓。为促进早产儿神经心理及体格发育，对其行干预治疗，评价干预疗效。方法：按照纳入标准收入病例80例，对纳入病例进行随机分组，试验组和对照组各40例。两组均予早期教育的基础上，试验组予益肾健脾按摩法，对照组予婴儿抚触法。分别在干预前，干预1个月结束、干预3个月结束时进行评测。主要评估体格发育指标，体重、身长、头围；神经心理发育指标，大运动、精细运动、适应能力、语言、社交行为。结果：益肾健脾按摩与婴儿抚触皆可促进早产儿体格发育，且益肾健脾按摩较婴儿抚触疗效更明显；促早产儿神经心理发育方面，目前二者差异无统计学意义。结论：益肾健脾按摩法作为一种符合小儿心身发育特点的早期干预方法，值得研究推广。 【关键词】 早产儿；早期干预；益肾健脾按摩；婴儿抚触；心身发育 Objective: For a long time follow-up visit, the premature would have more serious nerve system damage than the term infant. Also, not grow as well as them during infancy and school age. To improve the physical and intelligent development of the premature infant, we had taken an early intervention for them, and had an evaluation for the two different early intervention methods. Methods: A randomized controlled trial was conducted. 80 premature infants without Brain Damage Syndrome were collected, randomly divided into two groups. Each has 40 group members. The two groups both had early education as early intervention. The difference is that the experimental group received the treatment of Massage of Yishenjianpi when the control group received Infant Touch. They had assessments before intervention, after 1-month and after 3-month intervention. The assessment is physical development with weight, stature and head circumference; intelligent development with gross motor, fine motor, social adaptation, social intercourse behavior, language. Results: Both the two groups could improve the premature infant’s physical development. But the Massage of Yishenjianpi has a superior effect than the Touch one. For the intelligent development, the two groups have no significant difference. Conclusion: The Massage of Yishenjianpi based on the premature infant’s physical and intelligent structure is mean to have further study and good generalization.
