中文摘要目的研究中性粒细胞明胶酶相关脂质运载蛋白( Neutrophil Gelatinase-Associated Lipocalin, NGAL)、白细胞介素-18（Interleukin-18, IL-18）、半胱氨酸蛋白酶抑制 C（cystatinC, CysC）等肾脏损伤的生物学指标在冠状动脉造影（Coronary angiograph , CAG）介入（percutaneous coronary intervention, PCI）术后的变化及对对比剂肾病 ( Contrast Induced Nephropathy, CIN) 早期诊断的临床预测价值。方法选取我院 2012 年 5 月～2014 年 1 月期间 180 例行冠状动脉造影介入术治疗的患者为研究对象，检测术前、术后 1 天、2 天、3 天血 SCr、血 CysC、尿 IL-18 和尿 NGAL 的水平。发生 CIN 的病例 (以冠状动脉术后 48h 的血清肌酐（SCr）≥44. 2mmol/L 或升高≥25%为诊断标准)判为 CIN 组, 未发生 CIN 的病例判为非 CIN 组。结果1180 例行冠状动脉造影介入术治疗的患者中出现 13 例 CIN 患者，CIN 的发生率为 7.2%。2术后第 1 天尿 NGAL 水平达到高峰,较术前基线水平显著升高 (P0.05)。3术后第 2 天尿 IL-18 水平达到高峰,同基础水平相比有显著差异(P0.05)。4血 CysC 水平无明显性变化(P0.05)。5尿 NAGL, IL-18 在 ROC 曲线下的面积分别为 0.902、0.763，敏感性分别为 79%、64%，特异性分别为 58%、82%。结论1.尿 NGAL 是早期诊断急性肾损伤(AKI)的一种敏感的标志物。2.尿 IL-18 在早期预测 CIN 中也具有一定的诊断价值。3.尿 NGAL 较尿 IL-18 在预测 CIN 更加敏感，但 NGAL 单独诊断 CIN 受到许多潜在因素的干扰（如交叉反应物、内源性物质等）。关键词急性肾损伤；中性粒细胞明胶酶相关脂质运载蛋白；半胱氨酸蛋白酶抑物 C；白细胞介素-18：对比剂肾病。English AbstractObjectiveTo explore the significance of Neutrophil Gelatinase - Associated Lipocalin, Interleukin-18cystatin C, in early diagnosis of kidney damage in Coronary angiography after percutaneous Coronary intervention.Methods:Selected180 patients from January , 2014toMay, 2012 in order to detect the levels of NAGL in urine, IL-18 urine, cystatin C in blood and creatinine in blood, the samples ware collected in preoperative, The first day, the second day and the third day, depending on the level of creatinine the case ware distributed to the NO CIN group and the CIN group.Results180 cases of the object of study in 13 cases of CIN，the incidence of cin (7.3%).The level of NGAL in urine of the first day reaches its peak, it has statistically significant (P 0.05);.IL - 18 levels peaked at the second day (P 0.05).Blood CysC change is not obvious, there was no statistically significant difference (P 0.05).The area of NAGL, IL – 18 under the ROC curve were 0.902, 0.763, and the sensitivity was 79%, 64%, specificity of 58%, 82% respectively.ConclusionUr
