史垡旦膣匿堂盘查!!!!生!旦筮塑鲞增刊!g!垫!!!里!趔：垒!翘塾；Q!!：y!!：塑：!!PP!：!·15·．牙体牙髓病学研究．下颌恒牙主根尖孔偏移的体外研究陈晨姜岚高学军梁宇红【摘要】 目的观察下颌恒牙主根尖孔偏移根尖的位置和偏移距离，为临床根管治疗中确定工 作长度提供参考。方法利用立体显微镜观察162颗离体恒牙(前牙74颗，前磨牙46颗，磨牙 42颗，北京大学医学部解剖与组织胚胎学系提供)的194个主根尖孔开口的位置和方向，使用二维图 像测量软件(Image Measure，CF．2000C，上海长方光学仪器有限公司)在镜下使用显微标尺精确到 0．叭mm测量每个牙根的主根尖孔偏移解剖根尖的距离。结果44．3％(86／194)的下颌牙主根尖孔 偏移解剖学根尖位置，开口于根尖侧方，前磨牙根尖孔开口于侧方比例最高为56．5％(26／46)。下颌 牙主根尖孑L开口于远中根面比例最高，为15．5％(30／194)；14．9％(29／194)的根尖孔偏移开口于唇 侧和舌侧根面。根尖孔平均偏移距离为0．32 ram(0—1．88 mm)；磨牙组根尖孔偏移距离最大，平均 为0．37 mm(0～1．88 mm)；前牙偏移距离最小，平均为0．24 mm(0～1．20 mm)。结论临床进行根 管治疗时，应参考下颌恒牙主根尖孔开口于根尖侧方的比例(44．3％)及平均偏移距离(0．32 mm)，结 合多种手段准确进行治疗和评价。【关键词】牙根尖；解剖学，局部；主根尖孑L；根尖孔偏移An in vitro study on the position of the major apical foramen in mandibular permanent human teeth CHEN Chen，JIANG tnn，GAO Xue-jun．LlANG Yu—honb Department of Cariology and Endodontology， Peking University School and Hospital of Stomatology，Bering 100081，ChinaCorresponding author：LIANG Yu—hong，Email：leungyuhong@sina．corn，％Z：0086—10Abstract】 Objective To determine the location of the major foramen and the deviation distance from the anatomical root apex in human mandibular teeth．Methods A total of 162 madibular root specimens (1 94 major apical foramen)were examined under optical stereomicroscope．The distance from the anatomicalapex to the most occlusal point of the major foramen was measured using a micrometric SCale with an accuracy of 0．01 inn．The deviation of the major foramen from the anatomical apex was classified as central or lateral (buccal，lingual，mesial，or distal)．Results In 44．3％(86／194)of specimens，the major apical foramen deviated from root apex．The most frequent deviations of the foramen were in mandibular premolars 56．5％ (26／46)and to the distal 15．5％(30／194)．Fourteen point nine percent(29／194)of the major apical foramens were deviated t。the buccal and lingua|positions．The mean distance between the maior foramen and the anatomical root apex was 0．32 mm(range：0—1．88 mm)．The mean
