使用超声刀进行甲状腺手术探究[摘要] 目的：探讨使用超声刀进行甲状腺手术的适应证、方法、效果与并发症。方法：使用超声刀代替传统方法（结扎和缝合作为主要切割、止血的方法）对716例毒性甲状腺肿、甲状腺腺瘤、结节性甲状腺肿以及分化型甲状腺癌等多种甲状腺疾病，通过颈部小切口进行部分甲状腺切除术或腺叶全切术或根治性切除手术。 结果：手术平均时间72（42~134） min，术中平均出血量30 (6~130) ml，术后未出现大出血，术后切口引流量平均75(50~210) ml，术后抽搐及低钙血症者2例。4例出现轻度喉返神经麻痹症状，分别于2～6周恢复。术后平均住院时间4.5（4~6） d。结论：小切口超声刀甲状腺手术的手术时间短，术中、术后出血少，手术并发症少，术后恢复快，手术切口小，具有美容效果。改变了甲状腺手术依靠结扎和缝合止血的方法。 [关键词]超声刀；甲状腺；手术 [中图分类号]R653[文献标识码]A [文章编号]1673-7210（2008）07（c）-060-03 The study in thyroid surgery with ultrasonic LIANG Zhi-hong，TANG Zhi-ping，ZHANG Jin-cheng, WANG Hong-xia,GUO Zhou-qing （Department of General Surgery,Xiaolan People’s Hospital of Zhongshan City，Guangdong Province, Zhongshan 528415, China） [Abstract] Objective: To investigate the methods and results of ultrasonic dissection in thyroid surgery with small incision.Methods:Thyroidectomy,total or subtotal lobectomy of thyroid had been performed at 716 patients with thyrotoxicosis, thyroid adenoma, nodule thyrocele and papillary thyroid cancer by UAS in place of the conventional method such as deligation suture and hematischesis through thecervical minimal incision.Results:Mean operating time with the UAS was 72(42～134) minutes. The mean bleeding volume in operation with the UAS was 30(6～130) ml. No hemorrhea occured inpatients postoperatively. Mean scarf drainage in the first 24 hours postoperation was 75(50～210) ml. The postoperative low-grade laryngeal nerve palsy occurred in four patients and recovered after 2～6 weeks. The mean postoperative hospitalization time was 4.5(4～6) days.Conclusion:The use of UAS reduced operating time in thyroid surgery. The intra and postoperative bleeding volume and postoperative complication with the UAS were less than those with the conventional method. The patients recovered more quickly than those with the conventional method after operation. The scarf was very small and it has advantage of cosmetology.
