    Whenlaserwasfirstintroducedfordiaicmacularedema(DME),weweretaughtto

    “shootatthereddots.”Thereddotsaremicroaneurysms,andshootingatthemdirectly

    suggestedthatlaserworksbydirectcoagulation.Butovertheyears,physiciansbeganto

    noticethatedemawouldimproveeveniftheymissedthereddots.

    Usingtheargonlaser,withablue-greenwavelength,majorityoftheenergyisabsorbed

    bytheretinalpigmentepithelium(RPE).Thebloodinsidethemicroaneurysmsisnotwell

    absorbedbythelaser,andhencetheconceptofdirectcoagulationhasbeenboughtinto

    doubt.Furthermore,clinicalstudiesofthetreatmentofDMEcomparingargongreenand

    kryptonred(whicharenotabsorbedbyblood)showednodifferencesinefficacy.

    Therefore,itisunlikelythatdirectcoagulationisneededinreducingedema.

    TheconceptofmicropulselaseristodelivermoreenergytotheRPEcellswithoutthe

    collateraldamagetotheneurosensoryretina.Laserenergyisdeliveredinpulses.With

    eachspotof200msduration,therewouldbe100shortpulses.The“off”periodwithinthe

    pulseallowsthetissuetocooloff,andhencemoreenergycanbedeliveredtothe

    targetedtissue(RPE)withoutheatingthesurroundingneurosensoryretina.

    Wlishedoneofthefirstprospectiverandomizedcontrolledtrialssuggestingthat

    micropulselaserisasgoodasmodifiedETDRStreatmentbutwithlessscarring.

    Vujosevicandcolleaguesconfirmedourfindingsbutalsoshowedthatretinalsensitivityis

    significantlyterinpatientstreatedwithmicropulselaser.Furthermore,notonlywere

    thelaserscarsnotvisibleinclinicalexamination,butfundusautofluorescencenever

    changedinthemicropulsediodelasergroup.
