改良型 改良型 SBT 试验和不间断无创通气序贯在合并心功能不全机械通气患者脱机过程中的应用 PAGE PAGE 4 The application of improved SBT test and continuous noninvasive ventilation sequential treatment in using invasive ventilation for respiratory failure in patients with cardiac insufficiency ABSTRACT Introduction: With the progress of the aging society,the use of invasive ventilation for respiratory failure in patients with cardiac insufficiency had more and more be seen.Patients with congestive heartfailure or (and) ischemic heart disease using invasive ventilation had Longer ICU length of hospital stay and mechanical ventilation times than that other patients.Generally, Mechanical ventilation made negative effect to patients’hemodynamic with normal cardiac function.Because With each inhale,chest positive pressure would reduce venous returned blood volume. On the other hand, positive pressure ventilation could improve patientscardiac pulmonary edema and had been used in clinical treatment for routine treatment. Although cardiac function would be improved when making long-term mechanical ventilation, SBT test had changed the lung machinery mechanics and reduced pressure support and PEEP thus Leading to increased oxygen consumption and venous returned blood volume,which made it easier for the weaning associated pulmonary edema. At the same time, the strong evidence to support the use of noninvasive ventilation could largely prevent and treat acute cardiac pulmonary edema as well as reduce the weaning failure. Therefore the mechanical ventilation combined clinical heart failure patients’SBT test parameters setting need to make necessary adjustments and to assist continuous noninvasive ventilation sequential treatmentto increase the success rate of weaning. Objective: To explore the success rate of improved SBT test in heart failure patients with mechanical ventilation. Observe the efficacy ofcontinuous noninvasive ventilation sequential treatment in weaning and improvement of cardiac function. Method: Choos
