脑积水(Ⅱ):侧脑室―横窦CSF分流术治疗脑积水的实验研究 [摘要] 目的:研究侧脑室-横窦CSF分流术治疗脑积水的可行性,为临床治疗脑积水提供新的理论、实验依据和手术方法。方法:用家兔自体颈项部肌肉研浆枕大池注入法制作兔脑积水动物模型34只,并随机抽取分为实验组18只,对照组16只。3周后,实验组测量脑室压力后,行侧脑室―横窦CSF分流术,5周后再次测量脑室压力,取出分流管并检查其通畅性,而后断头处死,取全脑固定、切片,测量脑室大小。对照组除分流管用银夹夹闭不通外,其余和对照组完全相同。结果:a. 实验组动物术后次日食欲和食量均增加,精神萎靡和多卧症状消失,而对照组动物症状无明显变化;b. 术前两组之间mICP无显著性差异(P0.05),而术后实验组动物mICP明显低于对照组(P0.01);手术前后相比,实验组术后mICP明显低于术前(P0.01),而对照组手术前后无明显变化(P0.05);c. 处死前检查分流管实验组分流管通畅,横窦无血栓形成,对照组阻断彻底,横窦无血栓形成;d. 切片脑室测量显示,实验组脑室小于对照组,差异有极显著性(P0.01)。结论:侧脑室―横窦CSF分流术治疗脑积水方法可行,疗效好,建议进行临床实验研究。 [关键词] 侧脑室 横窦 分流术 脑积水 兔 Hydrocephalus(Ⅱ): A study of lateral ventricle—transverse sinus shunt in rabbits with hydrocephalus 作者单位: 056002 邯郸, 河北工程学院附属医院神经外科 Ma Dongzhou, Xu Liang, Sun Hongyan. Methods The models of hydrocephalus were made in 34 rabbits by cerebellomdedullary cistern injection of 2 ml of rabbit itself neck muscle solution. First, the ICP was tested and the lateral ventricle—transverse sinus shunts were inserted in all of 18 animals of experiment group at 3 weeks after muscle solution injection, and the animals were killed at 5 weeks after ICP tested. The ventricles were measured on the brain tissue slices. The same method was performed in control group, but the drainage catheters were hold with silver clips. Results a. The symptoms of increased ICP of the animals had disappeared after shunting in the experiment group, but it hadn’t in the control group. b. There was no significantly difference of mICP in the two groups before shunting (P0.05); but the value of mICP was significantly decreased in experiment group than in control group (P0.01).
