ResearchofSerumInterleukin-18LevelsandInflammationinpatientswithGoutAbstractObjective：ToexploretheeffectofIL-18onpathogenesisofgoutbydetectingtheIL-18levelsandIL-18mRNAexpressionfromperipheralbloodmononuclear(PBMCs)inpatientswithgout.Methods:Weselected40goutcasesand30healthycontrols,thecasesweresubmittedtoaneight-weekdecreaseduricacidinterview(FebuxostatTabletsorAllopurinol).Atonsetandafter8weeks,wemeasuredWaistcircumference(WC),height,weightandbloodpressure(SBP,DBP).TheserumofIL-18levelsweredetectedbycompetitiveenzyme-linkedimmunesorbentassay(ELISA).Wealsocheckeduricacid(UA),totalbillirubin(TBIL),triglyceride(TG),totalcholesterol(TC),Fastingplasmaglucose(FPG),lowdensitylipoprotein(LDL),highdensitylipoprotein(HDL),andfastingseruminsulin(FINS)andcalculatedHOMA-IR.PearsonscorrelationanalysiswasusedtoanalyzethecorrelationbetweenIL-18andothermetabolicparameters.IL-18mRNAexpressionfromPBMCsistestedbyRT-PCR.Results:⑴theIL-18mRNAexpressionwashigherincasesthancontrolgroup(P0.05).ThelevelsofWHR,BMI,SBP,DBP,UA,FINS,TC,TG,LDL,FBG,HOMA-IR,IL-18weresignificantlyhigherinpatientswithGoutthanthoseinthenormalgroup(P0.05orP0.01).HDLwassignificantlylowerinthegoutpatientsthanthoseinthenormalgroup(P0.05).⑵PearsoncorrelationanalysisindicatedthatserumIL-18levelswerepositivelycorrelatedwithUA,BMI,TG,FINSandHOMA-IRinthegroupofcase.⑶UAandLDLwerereduceaftertheintervention(P0.05orP0.01),whileserumIL-18levelswereincreasedaftertwoweeksofintervention,andweredecreasedafter8-week(P0.05).Andotherindiceswerenotsignificantlychanged.⑷ChangesinserumIL-18levelswerenotassociatedwithchangesinUAorLDL（P＞0.05）.Conclusion:TheIL-18mRNAexpressionandthelevelsofserumIL-18wereincreasedinpatientswithgout;decreaseduricacidinterventioncanchangethelevelofIL-18,indicatingthatgoutwascorrelatedwithinflammation.ThelevelsofserumIL-18correlatedwithinsulinresistance,whichmaybeinvolvedinthedevelopmentandprogressionofgout.Keywords:Gout;Interleukin-18;Inflammation;Insulinresistance.英文缩略表缩写英文名称中文名称HUAHyperuricemia高尿酸血症IL-18Interl
