Maintenance electrician (the victim) requested the opening of the locked roof access to Services Supervisor in coordination with the Electrical Supervisor to start work. At 10:00 am people near ?heard a noise and saw a person falling across the ceiling impacting on the floor.事故通告 Injury Notification事故介绍 Injury Description目前已知的原因如下（具体调查结果见后续正式的来自全球的安全警报）：具有工作许可证，但是对风险的评估不完善 ；现场没有监管，电气主管没有到现场，维修主管打开不安全屋顶入口后就离开了；推测电工没有使用合适的PPE，需要等待后续调查时进行确认。We know the following reasons, but the official investigation result will be released with safety alert.the task permit was done......but poorly assessed the riskthe task was not supervised on field.
