精品论文 参考文献 高频超声对高血压患者颈动脉结构与功能改变的评价 张加丰1 高学峰2（1黑龙江省中西医结合研究所附属医院 150090;2黑龙江省医院 150036） 【中图分类号】R544.1 【文献标识码】A 【文章编号】1672-5085 (2011)01-0205-03 【摘要】 目的 利用高频超声研究高血压患者颈动脉结构与功能的改变。方法 利用高频超声对46例高血压患者和40例健康人颈动脉做二维超声检查，观察并记录颈动脉内－中膜有无增厚、有无斑块、及其数目和回声情况。测量舒张期内－中膜厚度（IMT）;M－型记录颈总动脉（CCA）舒张期和收缩期的内径，计算内－中膜横截面积（IMSCA）、僵硬度指数beta;、扩张性和顺应性等参数。结果 高血压患者比健康人斑块检出率高，IMT增厚，IMSCA、收缩期内径和舒张期内径、僵硬度指数增大，扩张性和顺应性降低（Plt;0.05）。IMT与僵硬度指数、扩张性和顺应性无明显相关性。IMSCA、僵硬度指数、扩张性和顺应性与脉压有关（Plt;0.01）,而与收缩压和舒张压等无明显相关性（Pgt;0.05）。结论 高频超声是诊断颈动脉早期动脉硬化的简便有效的方法，IMT结合僵硬度、扩张性和顺应性等指标可反映高血压患者颈动脉结构和功能变化的特征。 【关键词】 高血压 高频超声 颈动脉 内－中膜厚度 僵硬度 扩张性 顺应性 High-frequency Ultrosound Evaluation of Alternations of the Struction and Function of Carotid Artery in Hypertention Groups ZHANG Jia-feng Harbin 150036,China 【Abstract】 Objective To study the alternations of the structure and function of carotid artery in patients with hypertention by high-frequency ultrosound .Methods The carotid artery of both 46 patients with hypertention and 40 subjects as normal control group were examained with high-frequency ultrasound .2-dimentional ultrosoud was used to observe the intima-media thickness(IMT),the echo and number of plaques.M-mode was used to measure the diameter of systolic and diastolic period and to calculate the cross-sectional area(IMSCA),stiffness index， distensibility and compliance.Result The number of plaques were higher in hypertention group than in the control subjects.The IMT ,IMSCA,systolic diameter and diastolic diameter were larger in hypertention group.There were higher stiffness index ,lower distensibility and compliance in hypertention group(Plt;0.05).IMT was not correlated to stiffness index, distensibility and compliance.IMSCA,stiffness index ,distensibility and compliance were correlated to pulsepressure(Plt;0.01),and not to systolic pressure or diastolic pressure.Conclusions High-frequency ultrosound is a convenient and effective method to evaluate early carotid artery atherosclerosis.
