Methods 50 cases of patients with both coronary arteriography and three lesions (43 males, 7 females), mean age of 55 years (36 ~ 74), including 15 cases of left ventricular ejection fraction less than 30%, 80% patients with angina CCS grade III-IV grade. 4 cases underwent left anterior and small external incision beating heart surgery without cardiopulmonary bypass, while other patients underwent conventional mid thoracotomy, hypothermic cardiopulmonary bypass, CABG+TMLR, and 90% of patients underwent internal mammary artery. The aortic balloon counterpulsation (IABP) assisted circulation was performed in 14 patients after surgery. TMLR uses a holmium laser machine (US Eclipse, TMR, System, 2000, 20W). On average, 3.8 cases were bridged and 35 by laser drilling. Results 50 operations were completed in one year, and good clinical effects were achieved. No death occurred. The operation not only relieved the symptoms of angina pectoris, but also improved the cardiac function. Conclusion the recent clinical results show that CABG+TMLR is a safe and effective method for the treatment of coronary heart disease, especially for severe, diffuse and distal lesions and severe cardiac dysfunction.
