PART 50 REVIEW;30 CFR § 50.10 Immediate notification. If an “accident” occurs, an operator shall immediately contact the MSHA District or Subdistrict Office having jurisdiction over its mine. If an operator cannot contact the appropriate MSHA District or Subdistrict Office, it shall immediately contact the MSHA Headquarters Office in Arlington, Virginia by telephone, at (800) 746--1553. ;Fines for New Emergency Response Act of 2006;Fines for New Emergency Response Act of 2006;A death of an individual at amine; An injury to an individual at amine which has a reasonable potentialto cause death; An entrapment of an individualfor more than thirty minutes; ;An unplanned inundation of amine by a liquid or gas; An unplanned ignition or explosionof gas or dust; An unplanned mine fire not extinguished within 30 minutes of discovery;;An unplanned ignition or explosionof a blasting agent or an explosive; An unplanned roof fall at or abovethe anchorage zone in active workingswhere roof bolts are in use; or, an un-plannedroof or rib fall in active workingsthat impairs ventilation or impedespassage;;A coal or rock outburst thatcauses withdrawal of miners or which disrupts regular mining activity for more than one hour;;An unstable condition at an impoundment, refuse pile, or culm bank which requires emergency action in order to prevent failure, or which causes individuals to evacuate an area; or, failure of an impoundment, refuse pile, or culm bank;;Damage to hoisting equipment ina shaft or slope which endangers an individual or which interferes with use of the equipment for more than thirty minutes; and;An event at a mine which causesdeath or bodily injury to an individualnot at the mine at the time the eventoccurs.;30 CFR § 50.11 Investigation.
