乳腺癌全数字化摄影的影像动态变化分析 [摘　要]　目的　探讨乳腺癌影像的动态发展演变规律,提高钼靶影像诊断准确性。方法 　应用主观评价方法及CAD软件系统回顾分析10例病程2月-2年，后经手术病理学证实的乳腺癌患者全数字乳腺摄影表现及动态演变。结果 ①肿瘤大小变化：全部病例肿瘤直径由小变大；②肿瘤密度：全部病例密度由低变高，由不均匀向均匀变化，监视器灰度平均值由由1589升至1821；③微钙化：微钙化4例由少变多，由稀疏变密集；④毛刺征：3例由无变有，4例由少变多；⑤边缘：由清楚变不清楚2例，⑥同侧淋巴结由正常大小变增大3例，⑦皮肤：2例出现皮肤增厚。结论 乳腺癌的动态演变规律为肿瘤由小变大，肿瘤倍增时间因肿瘤病理类型和分化程度而不同；微钙化由少变多，肿瘤在早期即可发生腋下淋巴结转移。 [关键词]乳腺癌；演变规律；放大摄影术；数字化乳腺摄影；计算机辅助诊断 Dynamic change of follow-up digital mammography studies in breast cancer patients 【Abstract】Objective To investigate the rule of dynamic development in the breast cancer mammography and improve the diagnostic accuracy of mammography. Methods 10 patients with full-digital mammography were analyzed retrospectively using subjective evaluation method and CAD software ，and dynamic evolution were analyzed also. All cases were confirmed by surgical pathology ，and their disease duration were 2 months to 2 years. Results ①changes in tumor size: tumor diameter in all cases from small to large; ②changes in tumor density: in all cases the density from low to high, dishomogeneity to homogeneity changes, the average gray-scale in monitors up to 1821 by 1589; ③changes in microcalcifications: 4 cases with little to much , change from the sparse to coarctation; ④changes in spicula: negative to positive in 3 cases, less to more in 4 cases; ⑤changes in Edge: clear to the intangibly; ⑥changes in axillary lymph nodes: ipsilateral lymph nodes size from the normal to increased in 3 cases; ⑦changes in skin： 2 cases occured thickened skin. Conclusion: The dynamic evolution of breast cancer is from small to big, the tumor doubling time depond on the pathological type and degree of differentiation; microcalcifications with little to much change, the tumor can occur axillary lymph node metastasis in early stage.
