重庆巨延百货 PAGE  附件二 重 庆 巨 延 百 货 装修管理规范 REGULATIONS OF DECORATION MANAGEMENT 第  PAGE \* MERGEFORMAT 12 页 共 8 页 For a good business environment can be provided to tenants , all the construction company that will construct in Ju Yan department ,please obey the regulative stipulations and requirements in Chonqing Juyan regulations of decoration managementand it will be carried out after promising and signing by the decoration unit. 为保障向商户提供一个良好的经营环境，凡进入巨延百货商场的装修单位，请严格遵守《重庆巨延装修管理规范》中的相关规定及要求，并由装修单位签章承诺后遵照执行。 一、Decoration of tenants 商户进行装修 Application should be submitted to property department first,after the approval form of decoration being reviewed and ratified,decoration work can be carried out. 应提前向物业部提出申请，详细填写《装修审批表》经审核、审批后方能施工。 二、Handling materials for decoration application 二、办理装修申请所需资料 1、Leasing party should declare to fire department and handle relative materials,and submit fire record certificate and approval opinion to property department. 由租赁方自行向消防主管部门申报办理相关手续，并向物业部提交消防备案凭证及消防部门批复意见一份。 2、Leasing party should declare procedures to relative department ,and submit a copy of procedures materials to property department. 2、由租赁方负责向有关部门办理各项报批手续，向物业部提交报批材料复印件一套。 3、Director of each decoration unit of tenants(non-tenants need to submit ID copy、proxy、trustee’s ID and its copy )should submit original and photocopy of business license and enterprise qualification、decoration drawing and after form of decoration application being confirmed,decoration permission can be got,then pay the decoration margin according to charge standard. 3、各商户（非租户办理需提供身份证复印件和委托书、被委托人身份证及复印件）装修单位负责人持装修单位的《营业执照》、《资质证书》原件及复印件（加盖公章）、装修图纸。填写装修申请经审批同意后，办理《装修许可证》，按照缴费标准交纳装修保证金。 4、 4、装修单位的施工人员凭身份证及复印件、一寸彩照一张申办《装修工人出入证》。该证必须随身佩带，专人专证使用。如有丢失，要及时向物管部申请补办，缴纳补办工本费。 5、物管部接到《装修申请表》及有关图纸后，3个工作日内作出审批意见。如报批图纸不符合消防安全以及相关管理规定造成装修审批延期，巨延百货不负责任何延期责任。 三、19.
