TST-Mega与PPH术治疗环状混合痔的临床疗效比较上海肛泰医院 (上海200023）尹吉庆1 王芳1江苏省徐州市中心医院肛肠科 （江苏 徐州221009）杨光2【摘要】目的：探讨大视窗选择性痔上黏膜切除术吻合器（TST-Mega）治疗Ⅲ-Ⅳ度混合痔的临床疗效。方法 将2014年8月-2015年7月期间收治的86例环状混合痔患者按随机序列表法分成两组，其中TST-Mega组（42例）与PPH组（44例），对比两组患者的手术情况、术后并发症和临床疗效。结果 TST-Mega组术后疼痛评分、平均住院天数均少于PPH 组（均Ｐ＜0.05），术后TST-Mega组疼痛感明显低于PPH 组（Ｐ＜0.05），TST-Mega组患者初次排粪急促感和吻合口狭窄的发生比例均明显低于PPH 组（均Ｐ＜0.05）；而两组的手术时间、术中出血量和术后尿潴留的发生率的差异无统计学意义（Ｐ＞0.05）。TST-Mega的治疗有效率为100％，PPH 组为97.7％，两组比较，差异无统计学意义（Ｐ＞0.05）。结论 TST-mega治疗Ⅲ-Ⅳ度痔安全、有效，操作简便，值得临床推广使用。【关键词】 环状混合痔；外剥内扎；外科肛管成形 Clinical comparison of TST-Mega and PPH in the treatment of Circumferential Mixed Hemorrhoids. Methods A total of 86 patients with circumferential mixed hemorrhoids from August 2014 to July 2015 were prospectively enrolled in the study and randomly divided into two groups , the experimental group (TST-Mega group) and the control group (PPH group). Surgical data, postoperative complications and efficacy were compared between the two groups. Results As compared with the control group, patients in the experimental group had a significantly lower postoperative pain and less hospital stay time, and also had some advantages in terms of postoperative complications such as the defecation feeling shortness and anal stenosis (P＜0.05).
