学 号：  HEBEI UNITED UNIVERSITY 毕业论文 GRADUATE THESIS 设计题目： 结核性脑膜炎脑脊液细胞学分析 学生姓名： 专业班级： 学 院： 指导教师： 2014年5月4日 结核性脑膜炎脑脊液细胞学分析 摘 要 目的：分析结核性脑膜炎脑脊液细胞分布情况，探讨脑脊液细胞学动态观察对结核性脑膜炎早期诊断及其预后评价的临床价值。 方法：收集30例结核性脑膜炎患者不同时期的脑脊液细胞学分类特点，并对其抗结核治疗前、治疗后进行细胞学动态分析。 结果：结核性脑膜炎患者脑脊液白细胞总数升高，多数患者早期呈典型的以嗜中性粒细胞为主的混合细胞学反应，经过有效抗结核治疗后，嗜中性粒细胞比例减少，淋巴细胞、单核细胞比例增多，呈以淋巴细胞为主的混合细胞学反应和淋巴细胞样反应，随着病情的好转，嗜中性粒细胞逐渐下降。 结论：脑脊液呈混合细胞学反应对结核性脑膜炎早期诊断有一定的价值，嗜中性粒细胞比例下降，淋巴细胞、单核细胞比例升高，提示抗结核治疗有效，脑脊液细胞学动态监测对结核性脑膜炎的预后评价有重要的临床意义。 关键词：结核性；脑膜炎；细胞学；脑脊液 Analysis of cerebrospinal fluid cytology of tuberculous meningitis Abstract Objective: To analyze the classification features of cerebrospinal fluid cytology of tuberculous meningitis; to explore the clinical value of cerebrospinal fluid cytology dynamic observation on early diagnosis and its evaluation of prognosis of tuberculous meningitis. Methods: A retrospective analysis of different stages of cerebrospinal fluid cytology classification characteristics of 30 patients who suffer from tuberculous meningitis; and cytologically analyze the patients’ cerebrospinal fluid cell before and after they receive the treatment of anti tuberculosis. Results: The number of cerebrospinal fluid leukocyte of the patients with tuberculosis meningitis increased ; in early stage most of the patients have mixed cellular reaction based on neutrophil , after effective anti TB treatment, the proportion of neutrophils decreased, and lymphocytes, monocytes ratio increased, with mixed cytology reaction mainly lymphocytes and lymphocyte reaction ; with the improvement of the disease, neutrophil decreased. Conclusion: Cerebrospinal fluid with mixed cytology reaction is of great value to early diagnosis of tuberculous meningitis; the percentage of neutrophils decreased, and the proportion of lymphocytes, mononuclear cells increased, this suggests that the effective anti tuberculosis treatment , and these have an important clinical sign
