Keuka College U.S.A Information for Application 美国库克大学入学申请须知 Please read the following instructions carefully before completing the application: 请在填写此表前认真阅读下列说明: 1.Application Form: Answer all questions completely, put “None” or “Not Applicable” where appropriate. Type or write clearly all required information and sign on the application form. 1.申请表：请完整填写申请表，对表中某些不适用于您的栏目，请注明‘None’或‘N/A’，填写所有信息必须字迹清楚，申请表必须签名。 2.TOEFL or IELTS: This is compulsory for all applicants whose mother tongue is not English. 2.托福或雅思成绩：对于英语为非母语的申请者，必须提供托福或雅思成绩。 3.Transcript of Academic Records: Photocopies of official seal transcript is required. Certified English translation must accompany documents which are not in English. 3.在校成绩单：必须提交盖有就读大学公章的成绩单复印件，如果是非英文的，请同时递交英文的正式翻译件。 4.Referee’s Report: You are required to submit three referee’s reports. It is strongly suggested that one referee is a Chinese teacher and two are foreign teachers who can comment on your management potential as well as your abilities in successfully completing the degree. Referee’s reports from relatives or personal friends will not be accepted. 4.推荐信：请提交3封推荐信，推荐人为一位中方老师，两位外教，他们须对您的管理潜能和成功完成学位的能力做出评价。亲属和私人朋友不得作为推荐人。 5. Referees are requested to enclose the report in an envelope, seal across the envelope seal. The sealed referee’s reports are to be attached with the application form or to be mailed directly to us. 5．推荐人必须将推荐信密封，在信封口签名。签封的推荐信必须和申请表格一起直接寄给我们。 6. Economic assurance or financial proof: Please submit documents proof personal finance or sponsored by family, government and financial warrantor with concrete number. 6．经济担保、财力证明： 请提交个人财力证明或家庭资助、政府资助、经济担保人赞助等方面的财力来源和具体金额。 7.
