构建我国“非营利性”医院的组织 框架 【内容提要】本文问题的提出基于 2000 年国务院《关于城镇医药卫生体制改革的指 导意见》决定在我国实行医院的“营利性” 和“非营利性”分类管理。论文阐释了“营 利性”与“非营利性”的概念内涵，追溯了 国外“非营利性”医院的历史渊源并简要 归纳其运作机制，在此基础上，分析“非营 利性”医院与我国原有的福利性质医院的 关键差别，指出我国目前医疗卫生机构改革 中，医院分类管理的核心问题是如何在政府 退出对医疗机构运作的完全责任的同时，保 证医疗卫生服务的公益性质。本文从“非营 利性”组织的特征与运作机制出发，试提出 我国医院“非营利”化的组织与制度框架。 关键词：非营利性，非营利性医院，医院分 类管理，医疗卫生机构改革 【Abstract】This article risen from the enaction of the Decision of the Reform of Medical Care System in Town in 2000, which divides China’s hospitals into profit and non-profit categories. The article expounded the concept of “profit”and “non-profit”, reviewed the history of non-profit hospitals in other countries, and summarized the characteristics of non-profit hospitals. Based on this, the article compared the “non-profit” hospital with the all-free hospital service in planned economy. It suggested that the key problem in classified management of hospitals in the reform of medical care system is how to keep hospitals public beneficial while the government failed to take all the responsibilities in the market economy. Based on the characteristic of “non-profit” organizations, this article try to give an organizational framework for China’s non-profit hospital system. Key words: non-profit, non-profit hospitals, classified management, reform of health care system 2000 年 2 月，为配合城镇职工医疗保险制度 改革，国务院办公厅转发了国务院体改办等 八部门《关于城镇医药卫生体制改革的指导 意见的通知》，明确我国医药卫生体制改革 的大政方针。8 月，卫生部及有关部门出台 13个配套文件，对卫生事业的补助、人事制 度、税收政策、医疗服务价格、药品相关问 题、社区卫生服务等医药卫生体制改革的各 个方面提供了操作规范。其中一个重要举措 即《关于城镇医疗机构分类管理的实施意 见》，决定进一步加大对医疗机构改革的力 度，实施医疗机构分类管理，在全国范围内 将医疗机构划分为非营利性与营利性两种 类型，分别实行不同的管理制度。 将医院划分为“营利性”与“非营利性” 进行管理，在我国医疗卫生体制中还是首次 采用。这次医疗卫生机构的分类管理改革具 有什么意义？什么是“营利性”与“非营
