腹腔甯支术在急腹症中的I鑰应用 2018-03-29 10:29:48中国现代医生2018年5期 朴光熙金红旭 ［摘要］目的探讨腹腔镜技术在急腹症中的临床应用。方法回顾性分析2016年1月?2017 年1月我院收治的60例急腹症患者的临床资料。40例行腹腔镜手术(腹腔镜组)，20例行开腹手 术(开腹组)，对两组的手术各项观察指标进行比较分析。结果腹腔镜组中，急性阑尾炎20例、 急性胆麥炎13例、粘连性肠梗阻5例、消化性溃疡穿孔修补术2例。开腹组屮，急性阑尾炎10例、 急性胆囊炎2例、粘连性肠梗阻2例、消化性溃疡穿孔6例。腹腔镜组患者的手术时间、术后下床 活动吋间及患者住院时间明显短于开腹组，术中出血量明显少于开腹组，术后并发症发牛率显著低于 开腹组，差异具有显著性(Pv0.05)。结论腹腔镜技术用于急腹症屮具有重耍作用，可显著提高疾 病诊断的准确率，且具有手术时间短、术中出血少、术后恢复快、并发症少、住院时间短等优点，值 得广泛推广和应用。 ［关键词］急腹症；腹腔镜技术；并发症；住院时间 ［中图分类号］R656.1 ［文献标识码］B ［文章编号］1673-9701 (2018) 05-0107-03 ［Abstract］ Objective To investigate the clinical application of laparoscopy in acute abdomen. Methods The clinical data of 60 patients with acute abdomen admitted in our hospital from January 2016 to January 2017 were analyzed retrospective!y. 40 cases underwent laparoscopic surgery (laparoscopic group) , and 20 cases underwent laparotomy (laparotomy group)? The surgical indicators between the two groups were compared? Results There were 20 cases of acute appendicitis, 13 cases of acute cholecystitis, 5 cases of adhesive intestinal obstruction and 2 cases of peptic ulcer perforation repair in the laparoscopic group? There were 10 cases of acute appendicitis, 2 cases of acute cholecystitis, 2 cases of adhesive intestinal obstruction and 6 cases of peptic ulcer perforation in the laparotomy group? The surgery time, postoperative ambulation time and hospitalization time in the laparoscopic group was significantly shorter than that of the laparotomy group. The intraoperative bleeding in the laparoscopic group was significantly less than that of the laparotomy group, and the incidenee of postoperative complications in the laparoscopic group was significantly lower than that of the laparotomy group, and the differenee was significant( Pv0?05 )? Conclusion Laparoscopic technique plays an important role in acute abdome n and can sign ificantly improve the accuracy of disease diagnosis .It has the advantages of shorter operation time, less intraoperative bleeding, faster posto
