Methods: We searched The Cochrane Central Register of Controlled Trials (CENTRAL)、PubMed、EMbase、CBMdisc、CNKI、VIP and web site of some clinical trials.Relevant journals were also hand searched to July 2012.We collected controlled trials comparing unicondylar medial arthroplasty(UKA) with high tibial osteotomy for medial compartment osteoarthritis of the knee .Study selection and quality assessment,data extraction and analyses were undertaken by two reviewers independently according to the Cochrane Handbook for Systematic Reviews of interventions.Meta-analyses were also performed. Results: 8 clinical trials involving 530 eligible patients were included, which contained 259 assigned to unicompartmental knee arthroplasty (UKA) and 271 assigned to high tibial osteotomy(HTO). The average age of the UKA group was 66.2 years (56-80 years old),while the HTO group was 60.8 years (53-78 years old).We compared the UKA group to HTO group from the outcomes, complications and revision, the meta-analysis results were as followed: the postoperative joint function excellent rate of UKA group was higher than that of HTO group, the difference was statistical significance (RR = 2.34, 95% CI 1.00-5.48, P = 0.05); compared UKA with HTO, a lower incidence of postoperative complications was found, the difference was statistical significance ( RR = 0.46,95% CI 0.24 -1.90, P = 0.02); although the UKA group had a lower rate of revision ,th difference was not statistical significance (RR = 0.74, 95% CI 0.36-1.52, P = 0.4). Conclusion: Both of the UKA and THO are useful and have their own techniques
