宁夏医科大学硕士研究生学位论文英文摘要 宁夏医科大学硕士研究生学位论文 英文摘要 Clinical Research on Laparoscopic Resection of Liver Neoplasm ABSTRACT Objective Application of laparoscopic techniques in liver surgery is another major breakthrough in minimally invasive surgery. Laparoscopic hepatectomy was firstly reported by Reich and co-authors and Gagner et al in the early1990-s. Since that time improvements in laparoscopic techniques and technological advances in instrumentation have significantly extended the surgeon ’s ability to perform the procedure safely. But many academics and physicians still held the suspicion attitude to curative effect. We used the selective laparoscopic hepatectomy and open hepatectomy to compare and investigate the effect of selective laparoscopi c hepatectomy for liver neoplasm. Methods This retrospective study enrolled ninety-three patients with hepatectomy between January 2008 and July 2012.Twenty-four cases of laparoscopic hepatectomy (group A), Sixty-nine cases of open hapatectomy (group B)for liver neoplasm were included. Subsequently, the influence of the two approaches on the parameters including intraoperative and postoperative situations, immediate and long term results was comparatively analyzed. Results Surgery were smooth completed. There are no statistics significance ( P0.05) of two groups between surgery time, blood lost, transfusion cases, maximum diameter of resection neoplasm, liver function, tumor recurrence rate, metastasis rate and mortality rate. but group A is superior to group B in post-operative hospital stays, feeding time, indwelling time of drainage tube and Analgesic usage, it has statistics significance(P0.05). compared the 1st, 3, 5 days of ALT, AST, ALB and TBIL of LH with OH, it has statistics significance(P0.05). Long-term efficacy of two groups have no statistics significance. Conclusions Short-term efficacy of laparoscopic hepatectomy is superior to open hepatectomy, long-term efficacy are similar.
