MCA 2005, Section 1. MCA Code of Practice, Chapter 3. Not achieved: Documented evidence is insufficient to indicate whether adequate steps were taken to communicate with the person. Needs improvement: There is some documented evidence of this, but insufficient detail to indicate whether all practicable steps were taken to communicate with the person. Fully achieved: There is clear documented evidence that all practicable steps were taken to communicate with the person, including steps taken to address any specific barriers to communication, e.g. use of translators, audiology, speech and language therapy, advocacy, involvement of family/friends etc. If there are any factors which might temporarily affect the persons ability to make decisions relating to their discharge (e.g. sedating medication, acute infection, mental health problems, emotional distress), appropriate steps have been taken to address these (e.
