As can be seen in this cumulative distribution curve, the late loss in Cohort B was much lower than what was seen in Cohort A, and is approaching the late loss seen with Xience V in the SPIRIT-First trial. The steepness of the red curve also illustrates how little variability there was in the distribution of late loss. There were interesting findings with IVUS. From post-procedure to 6 months follow-up, there was a nearly 12% reduction in ‘stent’ area, which accounted for the majority of late lumen loss. This was caused by the device not maintaining radial support in the vessel for long enough. But from 6 months to two years, the lumen size increased – not as a result of positive vessel remodeling, as there was no change in vessel area measured at the external elastic lamina (EEL). Rather, the increase in lumen size was the result of a decrease in the plaque/media layer of the wall. We are not certain what the mechanism was for this finding. We will be looking to see if this phenomenon occurs again in future trials, and if so, try to gain an understanding of exactly what is happening in the vessel. But this illustrates the unique and positive things that can potentially occur by restoring the vessel to a more natural state that is unconstrained by a permanent stent. In Cohort B, reductions in ‘stent’ area were minimal, resulting in a much lower late loss at 6 months than what was seen in Cohort A. It will be interesting to see if at 2 years we once again see late lumen e
