* LAM is effective in preventing pregnancy demonstrated in the high efficacy rates achieved through multicenter (11 site) trial as well as other clinical studies in the early 90’s. * LAM tested in clinical research studies with the 3 criteria depicted on the right, though to facilitate easier use and uptake by programs, the LAM working group proposed harmonization, simplificaton and operational definitons, with a focus on 2nd criteria – fully to nearly fully breastfeeding to breastfeed only. The last criteria was to encourage introduction of complementary foods. * LAM has mulitple benefits….Emerging evidence from the healthy fertility study indicate that within the context of LAM promotion, higher percentage of women exclusively breastfeed in intervention area (integrated MNCH care with FP), compared to a control group (MNCH) (p 0.01) * When ascertaining the reality of what is occurring in countires regarding LAM and breastfeeding practices, we turn to a 17 country analysis of Demographic and Health Survey data which shows prevalence of reported LAM use among women who are 3 to 6 months postpartum. At the high end is Uganda where more than 20 percent report use. At the low end are more than a half dozen countries where there is no reported use. * In several settings, Full BF is relatively high so LAM could be doable. This slide presents the percent of women who are FULL breastfeeding at 3 to 6 months postpartum. This slide is in stark contrast to the previous slide on LAM. This slide shows that at least in some countries a large proportion of postpartum women are full breastfeeding 3 to 6 months postpartum. We see that there is a huge range from almost 90 percent in Mali to less than 10 percent in Kenya. This difference begs the question: are women not incorporating breastfeeding into the use of LAM.
