汕头大学医学院硕士学位论文汕头大学医学院硕士学位论文 PAGE PAGE III ABSTRACT Objective To explore the role of matrix metalloproteinase-3(MMP-3) and matrix metalloproteinase-9 (MMP-9) in spondyloarthropathy (SpA) by investigating their expressions in serum and sacroiliac joint (SIJ) of SpA. Methods 50 cases with SpA were taken into account, including 31 cases were of ≥ grade Ⅱ and 19 cases were of grade 0 ～ I CT sacroiliitis. All patients were detected serum MMP-3 concentration and serum MMP-9 concentration and levels of erythrocyte sedimentation rate （ESR）and hypersensitivity C-reactive protein（hs-CRP）. 16 cases in the SpA were performed on CT guided needle biopsy of SIJ, including 7 cases were of ≥ grade Ⅱ and 9 cases were of grade 0 ～ I CT sacroiliitis. All SIJ tissue sections were stained with hematoxylin and eosin to obtain common pathological data, and immunohistochemical assay were performed to determine expressions of MMP-3 and MMP-9. Serum of 10 healthy volunteers and biopsy specimens of 2 non-rheumatisants served as healthy controls. Results Pathological observation showed that sacroiliitis were existed in all SIJ tissue specimens of SpA. Immunohistochemical study showed that both more MMP-3 and more MMP-9 were expressed in SIJ of SpA than those in healthy controls, and both more MMP-3 and more MMP-9 were found highly expressed in cytoplasm of inflammatory cells of pannus and bone marrow, as well as in cartilage cells. Furthermore, both the level of SpA serum MMP-3 and serum MMP-9 concentration were significantly higher than that of healthy controls. Conclusion Both MMP-3 and MMP-9 were highly expressed in the serum and sacroiliac joint in patients with SpA, suggesting they involved in the process of SpA disease. Particularly, serum MMP–3 may be a beneficial biological marker of SpA disease activity. Key words matrix metalloproteinases; spondyloarthropathy; serum; sacroiliac joint.
