2015年慢性病照護與自我管理國際研討會 2015 International Conference on Self- Management and Chronic Care 慢性病與多重慢性病為我國醫療照護系統中最重要的資源使用者，且隨著我國人口結構的逐年老化，多重慢性病的盛行率將逐年升高，其醫療照護模式之重要性也越顯提升。由政策面的角度而言，為提高慢性醫療照護品質及控制醫療成本的觀點，發展多重慢性病患照護模式實為當務之急。 據此，本研討會希冀以美國史丹佛病人教育研究中心之慢性病自我管理模式發展之經驗，與國內與慢性病照護領域相關之產官學專家學者共同分享討論，提升台灣在多重慢性疾病照護模式建立之基礎思維與能力，發展能夠有效提升健康、改善照護品質、降低醫療費用之多重慢性疾病照護模式，並增進參與者未來在相關政策規劃、照護模式設計實務工作之執行與推展，乃至慢性病照護議題研究能量之提升。 Patients with chronic diseases, especially with multiple chronic illness, often are the primary users of our limited medical resources. As the prevalence of the chronic diseases comorbidity increasesrapidly with aging population, the implementation of an effective chronic care model becomes more important nowadays. The development of chronic disease care model thus is the priority stage for enhancing quality of care as well as controlling medical cost. In this international conference, the development of chronic care and self-management model for comorbidity patientswill be well presented and discussed by the distinguished scholars from US, Japan, and Taiwan. Dr. Lorig, theDirector of Patient Education Research Center inStanford University will share their empirical experiences in the chronic care model.
